We are seeking an enthusiastic and experienced Band 7 Physiotherapist to join our Hospital at Home (H@H) Service in Swindon Intermediate Care Centre. This is an exciting opportunity to play a key role in the assessment, treatment, and rehabilitation of adults with complex physical and functional needs, supporting recovery and promoting independence. Working as part of a multidisciplinary team, you will provide expert clinical leadership, deliver high-quality patient-centred care, and contribute to service development within a dynamic intermediate care setting.

Main Duties of the Job:

• Joining the Swindon Intermediate Care Centre as a Band 7 Physiotherapist.
• Responsibilities include being part of the Hospital at Home Service, providing assessment, treatment, and rehabilitation support to adults with complex needs, and facilitating their safe return home.
• Working within a multidisciplinary team, offering clinical leadership and patient-centered care.

Main Responsibilities:

• Providing clinical support for the assessment, management and treatment of patients in community settings with particular focus on those referred into the service from the 2-hour Urgent Community Response (UCR).
• Coordinating the provision of specialist interventions to meet the urgent health care needs of people who would have traditionally been hospitalised or those requiring facilitated timely discharge from emergency departments.
• Demonstrating responsibility and accountability for the virtual ward clinical caseload and coordinating care across the whole patient/service user pathway.

Ideal Candidate:

• Registered with the HCPC
• Educated to degree level
• Recognised teaching or mentorship qualification
• Independent prescribing qualification
• Patient assessment qualification (such as PACR)
• Significant previous experience in elderly care, frailty, community services or relevant acute care specialty
• Clinical and technical skills in managing a range of clinical conditions
• Proven skills in managing complex patient/service user caseloads
• Experience of working as part of a MDT
• Excellent written and verbal communication skills
• Ability to confidently work autonomously and as part of a wider multidisciplinary team
• Strong judgement and analytical skills, with the ability to interpret complex situations and make appropriate decisions
• Ability to adapt to the demands of a constantly changing environment
• Good IT skills and knowledge of clinical systems
• Clean driving licence and access to a vehicle for daily use

Desirable:

• Experience of leading and managing a team
• Training in clinical supervision and/or leadership skills
• Involvement in relevant clinical interest groups
• Experience of clinical audit, research and understanding quality
• Experience in implementing change
