An exciting opportunity has arisen for an experienced clinical pharmacist to join the HIV and Sexual Health pharmacy team at Imperial College Healthcare NHS Trust. We are seeking a highly motivated, hardworking pharmacist committed to delivering high-quality, patient-centred care, who enjoys collaborative multidisciplinary working and is keen to develop specialist knowledge in a dynamic clinical environment.

In this role, you will provide clinical pharmacy support to general inpatient wards and to the HIV, HTLV and Sexual Health outpatient clinics. Working closely with the wider multidisciplinary team, you will offer expert medicines advice, support safe and effective prescribing, and help patients optimise their treatment. The post involves close collaboration with third-party outpatient pharmacy providers and Homecare services. This role offers an excellent opportunity to develop expertise in these areas and to make a meaningful contribution to individualising and optimising treatment. You will join a dedicated specialist team, with opportunities to develop your clinical expertise, contribute to training and education, and support the development of junior colleagues and pharmacy staff.

Main duties of the job

  • To support the delivery of a high-quality clinical pharmacy service to the HIV and Sexual Health specialty.
  • To collaborate effectively with members of the HIV and Sexual Health MDT, the wider pharmacy team and other healthcare professionals within and outside of the Trust, to provide person-centred pharmaceutical care to people living with HIV, HTLV and people accessing Sexual Health services.
  • To participate in HIV and Sexual Health MDT meetings including the HIV virtual clinic (HIV treatment and prevention), HIV MDT, Healthy Aging Clinic and HIV Inreach meeting, ensuring cost-effective and evidence-based use of medicines aligned with national guidance and commissioning policies where relevant.
  • To champion medicines management within the HIV and Sexual Health speciality.
  • To support audit, quality improvement initiatives, risk management and other elements of clinical governance within the specialty.
  • To attend and deputise for the Senior Lead and Lead Pharmacist at HIV and Sexual Health service, clinical pathways and business meetings and pharmacy departmental meetings.
  • To line manage and support the band 6 rotational pharmacists within the HIV and Sexual Health team.
  • To deliver and support education and training related to the HIV and Sexual Health specialty and the wider pharmacy department.

Person Specification

Education

  • Essential
    • Master's degree in pharmacy (or equivalent).
    • Registered as a Pharmacist with the GPhC.
    • Postgraduate certificate in pharmacy practice or equivalent qualification.
  • Desirable
    • Postgraduate diploma in pharmacy practice or equivalent.
    • Independent prescribing qualification (and registered as an independent prescriber with the GPhC).
    • Other related postgraduate qualification.

Experience

  • Essential
    • Broad hospital-based post-registration training and experience including all aspects of hospital pharmacy practice (medicines information, ward and dispensary services).
    • Experience of providing clinical advice and communicating therapeutic options clearly to prescribers and multidisciplinary teams.
    • Experience of working as part of a multidisciplinary team.
    • Experience of delivering education and training to pharmacy staff and/or other staff groups.
  • Desirable
    • Experience working within the HIV and Sexual Health specialty and MDT.
    • Experience of leading and undertaking clinical audit and/or quality improvement to improve practice.
    • Experience in setting up and managing clinical trials.
    • Experience in managing the supply of medicines through Homecare services.

Skills

  • Essential
    • Ability to identify and manage risks related to medication processes.
    • Strong clinical reasoning and professional judgement, including in complex or ambiguous situations.
    • Understanding of current NHS priorities, clinical governance and medicines optimisation.
    • Ability to identify and support the implementation of improvements to practice.
  • Desirable
    • Demonstrated good clinical knowledge in HIV and Sexual Health.
    • Ability to manage complex problems in HIV and Sexual Health.
    • Good understanding of how Homecare companies deliver services.
    • Good understanding of financial aspects of PbR excluded medicines.
