Job summary
We are seeking to recruit a positive, caring and self motivated individual to join the team at our urgent care centre. The urgent care centre is a collaborative service supported by the 3 local practices within our PCN providing on the day care for our combined population of 43,000 patients.
Patients will present with a range of acute minor illness conditions and you will be required to have a strong foundation of knowledge and expertise to provide a high quality patient assessment and plan of care.
Our Urgent Care Centre is based at Hythe Hospital. The role comes with supervision from a GP and you will be working in an experienced team of ANPs/ACPs, paramedics and GP registrars.
Main duties of the job
The successful candidate is to undertake the following responsibilities in delivering care -
- Deliver a high standard of care as an autonomous practitioner working independently & in conjunction with other healthcare professionals to assess, diagnose and treat the conditions of patients attending urgent care initiating direct referrals as appropriate to other members of the primary care team or to secondary care.
- Refer patients for radiology investigations such as x-rays & USS in line with policy & appropriateness.
- Order bloods & other pathology diagnostic testing.
- Prescribe independently but in line with the prescribing formulary following policy, NICE (national) & local clinical guidelines.
- Maintain accurate & contemporaneous health records appropriate to the consultation, ensuring the accurate completion of all documentation.
- Communicate effectively with patients &, where appropriate, family & carers regarding their physical health needs, findings and treatment choices.
- Have a knowledge of & ability to work to policies & procedures, including but not exclusive to confidentiality, safeguarding, information governance, infection control, risk management & chaperoning.
- Demonstrate personal accountability, emotional resilience & work well under pressure.
- Have an ability to maintain effective working relationships & to promote collaborative practice with all colleagues.
- Knowledge of when to seek advice & refer to a registered health care professional.
About us
Red and Green Practice has surgeries based in Hythe (Waterside) and Blackfield. Our urgent care service is based at Hythe Hospital, (next door to the Waterside site), and has been providing a collaborate on the day service for just over three years. We aim to support learning and development with regular inhouse education and supervision. The team also come together during the day providing exceptional peer support.

Qualifications
Essential
- The post holder will be required to be a fully qualified and registered Advanced Nurse Practitioner or be working towards the qualification or have experience working in an urgent care setting.
- Additionally you must be an independent non medical prescriber.
