We currently have an opportunity for Crisis Care Nurse (Band 5) in our Crisis, Assessment & Treatment Team in Ealing
The post holder will be responsible for delivery of a high quality mental health service for patients under the care of Crisis Assessment and Home Treatment Team (CAHTT) who require assessment by secondary mental health services for pathway planning, crisis intervention in the community, or gate keeping for the inpatient wards and community recovery house with the emphasis for potential providing home treatment.
CAHTT is dedicated to providing timely, flexible, and person-centered interventions aimed at preventing hospital admissions and readmissions. The team focuses on supporting individuals within their communities, tailoring care to meet their unique needs.
The role involves actively contributing to the continuous development and improvement of services, ensuring that effective care pathways and individualised care plans are in place to facilitate urgent and crisis care access. The post holder will champion evidence-based practices by offering clinical advice and support, serving as a role model, and delivering education and training as appropriate.
Main duties of the job
Key Tasks/Responsibilities
- To develop and maintain communication with individuals or groups of patients, carers, and staff about difficult or complex matters, overcoming any problems in communication in a difficult situation.
- To communicate with people in a manner which: is consistent with their level of understanding, culture, background and preferred ways of communicating; acknowledges the purpose of the communication; is appropriate to the context; encourages their participation; responds to communication of any kind from them.
- To improve communication through: changing the methods of communicating; changing the environment; modifying the content and structure of communication; reflection on the barriers to effective communication.
- To confirm that you have accurately interpreted communications.
- To provide feedback to other staff on their communication at appropriate times.
- To maintain accurate patient records consistent with legislation and, policies and procedures.

- Relevant post registration experience or applicable placement experience

- Post qualification or placement experience to include appropriate experience within an acute mental health care setting and / or experience within community, crisis care, or liaison psychiatry
