HCRG Care Group is launching a brand-new Intermediate Care and Reablement Service in Medway. We are looking for a Specialist Occupational Therapist to join the team and help people recover, regain independence and stay in their own homes.
You will assess, plan and deliver rehabilitation across community and intermediate care settings, lead therapy provision, and contribute to service development while supporting discharge planning and collaborative working with health and social care colleagues.
