This is an exciting opportunity to join the Kent Health Visiting Service as a Family Partnership Practice Lead (FPPL) to work within the Swale District. The FPPL has a key role in embedding the Family Partnership Model within Kent Health Visiting Services under the guidance of the Family Partnership Programme Manager. The Family Partnership Model is an innovative approach which is evidence-based and enables parents and families to overcome their difficulties, build strengths and resilience and fulfil their goals more effectively.

Working as part of the local Health Visiting team, the Family Partnership Practice Lead will facilitate the early identification of families in need of additional support, provide enhanced support while delivering the Family Partnership Programme. The leads will support the delivery of the Healthy Child Programme, manage a small caseload of families, supporting them ante-natally and until their infant is a year old.

Main duties of the job:

• The post holder will also have a lead role in embedding the Family Partnership Model within district health visiting practice as well as providing clinical supervision, fulfilling both leadership and some line management responsibilities to the team.
• You will contribute to a safe and effective operational running of the Health Visiting Service.
• The work with clients is intensive and challenging but provides a real opportunity to make a difference to their lives.
• You need to be a resilient and flexible Health Visitor, with excellent communication, team leadership skills, while acting as a role model to uphold high professional standards and promote high-quality, person-centred care.

Person Specification:

Qualification Essential:

• Level 6 Degree in Nursing or Midwifery as well as a Specialist Community Public Health Nurse with the Health Visiting Qualification.
• Registered Nurse or Midwife with the Nursing and Midwifery Council, with a current registration.

Experience Essential:

• Ability to work effectively within your area of practice, supported by relevant experience.
• Managing complex caseloads and supporting/mentoring others.

Knowledge and skills Essential:

• Have knowledge of up-to-date clinical practice by using up-to-date skills and adhering to evidence-based procedures and clinical effectiveness frameworks.
• Know when you are within your own limits and when to escalate.

Abilities Essential:

• Be adaptable in work as things change as part of working in a wider team.

Desirable:

• Have an awareness of developments within the NHS and National Service Framework.

Working Environment Essential:

• Have a full, valid UK driving licence issued by an EU country and the ability to travel independently.
