Senior Community Children's Nurse - Children's Continuing Care Abbey Wood

Job summary

An exciting opportunity is available for an experienced Community Children's Nurse to work in the Continuing Care Team as a clinical lead. This is a clinical role to manage the continuing care caseload, it is separate and apart from the undertaking of continuing care assessments and care package set up, the focus of the nurse will be to lead on areas related to direct clinical care.

Main duties of the job

• Ensuring the Continuing Care Team (0-18 years) provides a high quality, evidence based locally appropriate service to families.
• Identifying new theories, policies and guidelines relating to complex health needs care in the community; disseminating research findings and developing local policies and standards accordingly.
• Providing professional and clinical leadership to all children's community nursing staff, ensuring that they are engaged with and adhering to local policies, guidelines and expectations.
• Assessing nursing care needs, developing, implementing and evaluating programmes of nursing care for children and young people on the caseload.

Job responsibilities

The role will be integrated within the existing Community Childrens Nursing Team and the Childrens Continuing Care Team, who are part of an integrated network of services for children and young people in Bexley and Greenwich providing high quality specialist child centred care. The team of specialist and generic childrens community nurses provides skilled, high quality, comprehensive nursing care to children and families with nursing needs in their own homes or community settings.

The post holder will be responsible for contributing towards ongoing developments of clinical practice, research and standards of care within the service, including policies, protocols, guidelines and collaborate with the wider multidisciplinary team implementing acute pathways between the hospitals, GP and diagnostic hubs into the community.

The post holder will provide a high standard of evidenced based nursing care for children and young people with the following conditions: children with disabilities and complex conditions including those requiring palliative and end of life care. To provide clear visible leadership supervision, caseload management and professional clinical expertise to the clinical skills nurse trainers in the team, to ensure every aspect of the child and families experience is delivered in a safe, efficient and evidence-based manner.

Education

• Registered children's nurse with evidence of continuing professional development in specialist area

Experience

• Knowledge of the National Framework for Children and Young People's continuing care

Desirable

• Experience in managing a complex caseload

Knowledge

• Knowledge of current practices in caring for children and young people with complex needs

Skills

• Skills in teaching junior staff members, and managing performance
