Advanced Clinical Practitioner Selby

Selby Town PCN is looking to recruit an enthusiastic, compassionate and motivated ACP to join our welcoming and highly supportive team on a fixed-term contract until the end of this financial year (31/3/2027) to support winter resilience. The successful candidate should be an experienced practitioner who, acting within their professional boundaries and competencies, will provide care to the presenting patient, from initial clinical assessment, through diagnosis, treatment, and an evaluation of care. They will demonstrate safe and effective clinical decision making and critical thinking. Selby Town PCN has a proven history of developing a variety of roles within the PCN & in coordinating & improving services for patients. Our locality network is committed to developing a multidisciplinary workforce which it sees as essential in providing an elevated level of care for its population. The PCN has 4 local practices, Beech Tree, Posterngate, Scott Road and Escrick surgeries serving more than 50,000 patients. This role is employed by Selby Prime Care Ltd, the employing and contracting company for Selby Town PCN, delivering collaborative healthcare services across our four member GP practices. You would work amongst a vibrant multidisciplinary team including GPs, ANPs, Paramedics, Pharmacists, Nurses, HCAs, all supported by superb administration and reception teams. The core objective of the ACP is to assist practices with the demands over the winter months.

Key Responsibilities:

• Assess, diagnose, and treat patients presenting with an undifferentiated diagnosis.
• Consider the holistic biopsychosocial needs of patients and plan clinical care accordingly.
• Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based national and local protocols, and within scope of practice.
• Recognise and work within own competence and professional code of conduct as regulated by the NMC or HCPC.
• Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures.
• Deliver care according to NSF, NICE guidelines, and evidence-based care.
• Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation.
• Evaluate patients' response to health care provision and the effectiveness of care.
• Support and participate in shared learning across the Primary Care Network.
• Use a structured framework (e.g., root-cause analysis) to manage, review and identify learning from patient complaints, clinical incidents, and near-miss events.
• Act as a clinical leader in the delivery of services ensuring that the needs of the patient are a priority.
• Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role model.
• Support staff development to maximise potential.
• Contribute to the development of local guidelines, protocols and standards.
• Undertake mentorship for more junior staff, assessing competence against set standards.
• Disseminate learning and information gained to other team members to share good practice and inform others about current and future developments (e.g., courses and conferences).
• Assess own learning needs and undertake learning as appropriate.
• Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.

Person Specification:

• Essential: Significant post-registration clinical experience, including recent experience working autonomously in an advanced or senior clinical role. Experience of assessing, diagnosing and treating patients with undifferentiated or urgent presentations. Experience of working within professional boundaries and escalating appropriately. Experience of accurate clinical record keeping and working within local and national policies and guidance.
• Desirable: Experience in primary care, urgent care, general practice or a Primary Care Network setting. Experience of working across multiple sites or within a multidisciplinary team. Experience of audit, quality improvement, service development or clinical supervision.

• Essential: Registered healthcare professional with current NMC or HCPC registration. Advanced Clinical Practitioner qualification or evidence of working towards an accredited advanced practice qualification. Independent prescribing qualification, or willingness and eligibility to work towards this if required for the role. Evidence of continuing professional development relevant to advanced clinical practice.
• Desirable: Masters level qualification in Advanced Clinical Practice. NHSE Centre for Advancing Practice recognition/digital badge or working towards this. Teaching, mentorship, supervision or assessor qualification.

• Essential: Advanced clinical assessment, history taking, examination and clinical decision-making skills. Ability to manage patients with acute illness, minor illness, long-term conditions and complex needs within scope of practice. Knowledge of evidence-based practice, NICE guidance, clinical governance, safeguarding, infection prevention and information governance. Ability to assess and manage clinical risk and prioritise workload safely.
• Desirable: Knowledge of local primary care pathways, medicines optimisation and population health priorities. Experience using primary care clinical systems, such as SystmOne or EMIS. Additional relevant clinical skills, such as minor illness, triage, home visiting or care home assessment.
