As a Community Nurse Band 5, you will deliver high-quality, person-centred nursing care to patients in their own homes, working in line with national and local policies, professional standards, and the NMC Code. You will work closely with patients, families, GPs, and multidisciplinary colleagues to assess needs, plan and deliver care, promote independence, and support positive health outcomes.

Main Responsibilities:

• Assess, plan, deliver, and evaluate nursing care in partnership with patients, carers, and the multidisciplinary team.
• Monitor patient progress, recognising and escalating changes in condition appropriately.
• Prioritise and manage a caseload in line with patient need.
• Maintain accurate and timely patient records using electronic and paper-based systems.
• Deliver evidence-based care and contribute to quality improvement initiatives and clinical audits.
• Promote health, wellbeing, and independence for patients and carers.
• Participate in team meetings, handovers, clinical supervision, appraisals, and mandatory training.
• Support the learning and development of students and colleagues.
• Contribute to maintaining a safe environment through risk assessment, incident reporting, and adherence to infection prevention and control standards.
• Ensure the safe use, maintenance, and management of clinical equipment and stock.

The Ideal Candidate:

You will be a NMC/1st level registered Nurse, with strong communication and organisation skills and a flexible approach. You will have a full UK driving license with access to a car for work purposes. You will also need to be confident using IT systems and demonstrate working knowledge of Microsoft Office packages.
