PAR Health Primary Care Network (PCN) is seeking enthusiastic, compassionate and forward-thinking GPs to join our supportive and innovative team. R Health PCN is a progressive and collaborative network of six practices serving a population of over 67,000 patients across Runcorn. Successful applicants will receive ongoing mentorship and development within your host practice, alongside additional support from GP Health Connect Ltd (GPHC), your employing organisation. Applicants must meet the current NHS England Additional Roles Reimbursement Scheme (ARRS) eligibility requirements. In line with the Network Contract DES, applicants must not have been substantively employed as a General Medical Practitioner in a Core Network Practice within R Health Primary Care Network at any time during the previous 12 months.
Main duties of the job
- Provide high-quality, patient-centred care within the aligned practices.
- Work collaboratively with a multidisciplinary team, including nurses, pharmacists, mental health practitioners, first contact physiotherapists and social prescribers, to ensure integrated patient care.
- Conduct consultations, assessments, and follow-ups with patients.
- Contribute to the management of chronic disease, health promotion, and disease prevention in line with best practices and PCN priorities.
- Participate in clinical audits, training, and quality improvement initiatives to enhance patient care within the PCN.
- Support the development and implementation of new care pathways that align with ARRS objectives.

The PCN consists of 6 practices and covers a population of just over 67,000 patients.
You will be employed by the GP Federation, GP Health Connect Ltd. The Federation employs ARRS staff on behalf of the PCN and delivers services across Runcorn within the practices and community venues. Job responsibilities
The post-holder will provide general medical services managing a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Clinical Responsibilities
- In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties including surgery consultations, video consultations, telephone consultations and queries, visiting patients at home, ward rounds, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
- Screening patients for disease risk factors and early signs of illness.
- In consultation with patients and in line with current practice disease management protocols, developing care plans for health.
- Providing advice and health education.
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
- Recording clear and contemporaneous consultation notes to agreed standards.
- Collecting data for audit purposes.
- Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).
- Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.
Other Responsibilities
- Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health, and safety.
- A commitment to life-long learning and audit to ensure evidence-based best practice.
- Contributing to evaluation/audit and clinical standard setting within the organisation.
- Contributing to the development of computer-based patient records.
- Contributing to the summarising of patient records and coding patient data.
- Attending training and events organised by the practice or other agencies, where appropriate.
- Contributing to teaching where appropriate.
- Attending practice team meetings.
General Duties
The above responsibilities will be undertaken upholding the following:
- To be available to take over the responsibility of patient care from the Out of Hours service at 0800 or where applicable maintain responsibility till the appropriate time when the Out of Hours service takes over care in the evening.
- To continue to work until all clinical tasks have been completed. To arrange with others to cover when planning to leave before the end of the session.
- At all times to behave in a professional way that encourages quality care and the development of a team spirit. 360-degree feedback is used to assess clinicians and you will be required to participate in this.
Communication and Relationships
- Communicate effectively with patients and carers and recognize people's needs for alternative methods of communication and respond accordingly.
- Develop and maintain professional relationships with all key stakeholders e.g. Practice team members, PCN colleagues, Federation, Community Nursing Teams, Community Mental Health Care Teams, Social Services, statutory and voluntary services.
Safeguarding
All members of staff have a responsibility to ensure the Safeguarding of patients. Ensure knowledge of Safeguarding policies and training is up to date. Demonstrate due regard for safeguarding and promoting the welfare of children.

In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. In the performance of the duties, the post-holder will have access to confidential information relating to patients and their carers, member practices, staff, and other healthcare workers. All such information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the organisation may only be divulged to authorised persons in accordance with the policies and procedures relating to confidentiality and the protection of personal and sensitive data. Health & safety
The post-holder will ensure the promotion and management of their own and others' health, safety and security as defined in the organisations Health & Safety policy and the Infection Control policy and published procedures. This will include (but will not be limited to):
- Ensuring adherence to individual responsibilities for infection control and health and safety, using a system of observation, audit and check, hazard identification, questioning, reporting and risk management.
- Maintaining an up-to-date knowledge of health and safety and infection control statutory and best practice guidelines and ensure implementation of these across all areas.
