You will aid in medicines reconciliation after hospital discharge, monitor high-risk medications, and contribute to data-driven improvement across the network.
You should be a GPhC-registered Pharmacy Technician, with preferred experience in EMIS or SystmOne, and a proactive, patient-centered approach.
