Job Summary

Experienced Practice Nurse (Full or Part Time) required to join a large GP practice in Paxton Green. The role supports a multidisciplinary team, including GPs, Physician Assistants, Clinical Pharmacists, a Social Prescriber, MSK Physios and a Drug Counsellor.

Main Duties

  • Run independent LTC clinics (e.g., Diabetes, Asthma)
  • Provide cytology services
  • Perform wound care, suture removal and venepuncture
  • Manage chronic diseases
  • Administer routine adult and childhood immunisations
  • Administer and manage SMI depot medications
  • Conduct home visits for vaccination and LTC reviews
  • Collaborate closely with MDT colleagues (clinical pharmacists, physician assistants, care coordinators) on diabetes care and other specialties
  • Work extended hours, including Saturdays, on a fair‑share rota (9 a.m.–4 p.m.)

Salary

£44,186 to £49,026 per year for full time (37.5 hrs) plus NHS pension and PGGP pay scale (grade F).

Job Responsibilities

  • Support patients with long‑term conditions in face‑to‑face consultations and independent clinics
  • Provide dressings, immunisations and general treatment room services
  • Carry out health screening, NHS health checks and holistic health assessments
  • Maintain accurate records and statistical reporting
  • Communicate abnormal findings to the nursing team or GPs
  • Conduct home visits as required
  • Engage in health promotion and patient education
  • Maintain competency under NMC code and practice policies
  • Attend primary health‑care team meetings and other relevant meetings

  • Adhere to NMC professional conduct and scope of practice
  • Take responsibility for own professional development and educational updates
  • Participate in team development activities and performance reviews

  • Thorough induction for new staff
  • Regular appraisals and access to training and development
  • Daily and weekly nursing meetings; senior support and monthly practice development forums
  • Clinical supervision opportunities

Maintain strict confidentiality of patient, staff and practice information in accordance with internal policies.

  • Use personal security systems and infection control procedures
  • Identify and manage risks associated with work activities
  • Report potential risks and hazards

Disclosure Barring Service (DBS)

Exempt from the Rehabilitation of Offenders Act 1974. A satisfactory DBS disclosure is required.

Person Specification

Qualifications

  • Registered General Nurse (RGN)
  • Practice Nurse qualification
  • Optional: Diploma or equivalent in diabetes and/or asthma & COPD care

Experience

  • Minimum 2 years as a practice nurse in general practice
  • Experience working within a large multidisciplinary team
  • Experience in health promotion
  • Demonstrated ability to work autonomously while recognising limits of competence
  • Desirable: 3+ years experience as a practice nurse; prior delivery of diabetes or asthma & COPD clinics

Skills

  • Computer literacy
  • Excellent interpersonal and communication skills
