The Confederation, Hillingdon CIC is seeking an experienced General Practice Nurse to join the North Connect PCN in Northwood. This part-time role (25 hours per week) is permanent and based at Eastbury Surgery.
The post holder will provide advanced nursing care for long-term conditions, lead within the nursing workforce, and contribute to population health management and health inequalities reduction. NHS values and professional standards guide daily practice.
