The Community Integrated Assessment Team (CIAT) at Stockton have an exciting opportunity for an experienced Band 6 Occupational Therapist/Physiotherapist looking to develop their leadership skills and/or a current Band 6 Occupational Therapist/Physiotherapist looking to develop and explore their skills and knowledge within community therapy services. The successful candidate will join a team aimed at providing therapy intervention to those patients that require support to regain or maintain their independence. The CIAT aims to support timely hospital discharge and prevention of admission to hospital or a care setting.
It is expected that the successful candidate will undertake a senior therapist role within the team, being responsible for their own caseload and the supervision of post graduate therapists and therapy assistants within CIAT. The successful candidate will also work alongside the Clinical Care Coordinator and participate in the planning, co‑ordinating, delivering and evaluating service development initiatives, such as the introduction of Community OPTICA and developing skills in clinical triage within iSPA.
Main duties of the job include working within the community therapy services across Stockton and Hartlepool. CIAT currently provides a seven day service and offers an extended service till 7pm Mon‑Sun. The post holder will work within CIAT delivering therapy across the areas which may include community rehabilitation, urgent community referrals, rehabilitation day unit, residential rehabilitation units additional community beds and iSPA.
The post involves working in partnership with all stakeholders including partners in health and social care to ensure the provision of high quality care delivery to this group of patients and their carers.
Responsibilities of the role will include a comprehensive assessment to aid in the reablement of the patient either at home or in a rehabilitation unit. Attending urgent referrals, assessing the patient working towards hospital avoidance and care closer to home. Health promotion/prevention: Maintain a focus on achieving and demonstrating overall improvements in health care, prevention of inappropriate admissions and timely discharges from hospital.
To provide people who live in Stockton and Hartlepool timely and appropriate therapy interventions in partnership with the multi disciplinary and multi agency team and/or provide rehabilitation needs identification and liaison with appropriate Community Services. Assessment of peoples needs and the prescription/provision of appropriate equipment/adaptations to enable their safe return to their lifestyle of choice.
To take a lead role in the advanced assessment and treatment of patients within the speciality who may have complex and/or chronic presentation, and to determine clinical diagnosis and appropriate management plan, and to maintain records as an autonomous practitioner. To treat patients in a variety of locations including emergency/urgent care, outpatients, community, intermediate care, residential/nursing homes and in the patients own home.
Liaison to Rapid Response/Intermediate Care and medical services to ensure the person returns from Accident and Emergency to a safe environment or to provide a functional perspective in Accident and Emergency which will support decision for admission or discharge.
Qualifications:
- Essential: HCPC registration, Qualified Occupational Therapist/Physiotherapist
- Desirable: Evidence of undertaking post graduate training

- Essential: Clear evidence of leadership experience, Evidence of mentorship/supervision, Evidence of service development/quality improvement

- Essential: Experience of working in a therapy based community setting, Post graduate experience which supports progression into senior therapist role

- Essential: Knowledge of government initiative affecting the delivery of community services in health care
- Desirable: Knowledge of intermediate care services
