Nurse Practitioner - Primary Care Mental Health Tamworth

Job summary

We are pleased to be able to offer an opportunity for an experienced mental health nurse and independent prescriber to take on an exciting new role in the Primary Care Mental Health Team.

This role will involve working in partnership with local GP practices to ensure reviews of prescribed medications are taking place for people who are under the Essential Shared Care Agreement (ESCA) with their GPs.

The role includes supporting GP practices to monitor prescribing and supporting joint working between the Integrated Mental Health Team (IMHT) and the GP Practices.

This is an extension of an existing programme of work providing joint physical and mental health reviews in local GP practices for people with a diagnosis of severe and enduring mental illness (SMI).

The role will include driving quality improvements through leading on areas such as patient satisfaction, data recording and reporting, clinical supervision, clinical audit and ensuring adherence to evidence-based practice.

Relationship building with Primary Care Networks and GP practices will form an important part of this role. An understanding of these relationships and an approach that supports collaborative working is crucial for the post holder.

Significant experience in mental health particularly working with people with severe and enduring mental health needs will be required to undertake this role.

Main duties of the job

• Work predominantly in GP practices in South East Staffordshire to provide assessment and review for individuals presenting with mental health needs.
• Review prescribing in collaboration with GP practices.
• Carry out assessments of service users following agreed practice guidelines.
• Implement and review care planning and risk management.
• Provide staff supervision to aid service provision for individuals presenting with mental health needs.
• Deliver a high quality clinical service to service users, families and carers in the community.
• Demonstrate an attitude which respects the core values of the organisation and the principles of recovery.

The team is looking for an individual who is enthusiastic, passionate, motivated and one who is willing to make a difference to their community, supporting individuals with their mental health and physical health care needs.

The successful candidate will be self-motivated and able to work in a fast-paced environment. You will be able to work autonomously and as part of a team. There is expectation to deliver review appointments within primary care facilities and therefore access to suitable transport during work hours is essential.

Experience

Essential

• Significant demonstrable experience of assessing mental health needs
• Experience of working with a range of statutory, non-statutory agencies
• Experience of working with more complex and challenging needs
• Evidence of experience in education of nurses and / or multi-professional groups

Desirable

• Experience of working therapeutically in community settings
• Experience of supervising junior staff

Qualifications

Essential

• A qualified/registered mental health nurse
• Non-Medical prescribing V300
• Additional post registration training
• Mentorship Course and supervision training

Desirable

• Masters level qualification in a relevant field, e.g. advanced clinical practice.
• Specialist Mental Health Training eg. Assessment
• Leadership Course

Skills and Knowledge

Essential

• Knowledge of various approaches to mental health assessment
• Knowledge/awareness of relevant legislation and guidance
• Ability to work collaboratively with other agencies.
• Ability to challenge and change clinical practice of self and others.
• Skills in inputting into and retrieving information in electronic health care records

Desirable

• Awareness of basic research methodology
