Job summary

We're looking for an enthusiastic and experienced Advanced Nurse Practitioner to join our friendly, ambitious and supportive team at Woodlands Park Health Centre.

This is an excellent opportunity to work in a well-established, forward-thinking GP practice where you'll be trusted to use your advanced clinical skills, work autonomously and play a key role in delivering high-quality care to our patients.

Ideally, we are looking for a candidate who is able to work on

Mondays and Wednesdays , although some flexibility may be considered for the right applicant.

As part of our multidisciplinary team, you'll work alongside experienced GPs, Advanced Nurse Practitioners, Practice Nurses, Pharmacists, Physician Associates and a skilled administrative team in a collaborative environment where everyone's contribution is valued.

Main duties of the job

As an Advanced Nurse Practitioner, you will be a valued member of our multidisciplinary team, delivering autonomous, patient-centred care across a wide range of acute, urgent and long-term conditions.

You'll assess, diagnose and manage patients, providing safe, evidence-based care while working closely with GPs and other healthcare professionals to ensure the best possible outcomes.

This varied role offers the opportunity to use your advanced clinical skills, contribute to service development, support health promotion initiatives and help shape the future delivery of primary care within our practice.

Job responsibilities

As an Advanced Nurse Practitioner (ANP) at Woodlands Park Health Centre, you will play a key role in delivering high-quality, patient-centred care as part of our multidisciplinary primary care team.

You will work autonomously within your scope of practice while collaborating closely with GPs and other healthcare professionals to provide safe, effective and evidence-based care.

Main Duties and Responsibilities

• Independently assess, examine, diagnose and manage patients presenting with undifferentiated and undiagnosed conditions.
• Undertake telephone, face-to-face and home visit consultations where appropriate.
• Develop, implement and evaluate evidence-based treatment and management plans.
• Prescribe safely and effectively as an Independent Prescriber, in accordance with national guidance and local policies.
• Request, interpret and act upon diagnostic investigations, including pathology and imaging results.
• Recognise and appropriately manage medical emergencies, escalating or referring patients when required.
• Refer patients to secondary care, community services and other healthcare professionals using appropriate referral pathways.
• Promote health, wellbeing and disease prevention through patient education and lifestyle advice.
• Manage patients with both acute presentations and long-term conditions, ensuring continuity of care where appropriate.
• Maintain accurate, timely and comprehensive clinical records using Systm One.
• Participate in clinical governance activities, audits, quality improvement projects and significant event reviews.
• Contribute to the development of practice protocols, policies and clinical pathways.
• Support the education and development of colleagues, students and trainees where appropriate.
• Work collaboratively with the wider multidisciplinary team to ensure high standards of patient care and service delivery.
• Comply with all professional, organisational and statutory requirements, including safeguarding, infection prevention, information governance and health and safety policies.
• Professional Responsibilities
• Practice in accordance with the Nursing and Midwifery Council (NMC) Code of Conduct.
• Maintain professional registration, revalidation and continuing professional development.
• Work within the scope of their competence and seek advice where appropriate.
• Demonstrate excellent communication, leadership and clinical decision-making skills.
• Promote equality, diversity and inclusion, treating all patients and colleagues with dignity and respect.
• Contribute to maintaining a positive, supportive and collaborative working environment.

Qualifications

Essential

• Registered Nurse with current registration with the Nursing and Midwifery Council (NMC).
• Recognised Advanced Nurse Practitioner (ANP) postgraduate qualification (Master's level or equivalent).
• Independent/Supplementary Prescriber (V300).
• Evidence of ongoing Continuing Professional Development (CPD).
• Evidence of advanced clinical assessment and decision-making skills.

Desirable

• Experience participating in clinical audit and quality improvement projects.
• Experience supporting students or mentoring colleagues.
• Experience managing patients with long-term conditions.
• Disclosure and Barring Service Check
