An exciting opportunity has arisen for a Band 5 Mental Health Practitioner to join the Adult Crisis Resolution and Home Treatment Team based at the Bradgate Mental Health Unit. We are looking for candidates who are either a Registered Professional Mental Health Nurse, Social Worker or Occupational Therapist who have completed their preceptorship.
The Adult Crisis Resolution and Home Treatment Team provide a service for adults aged 18 and over who, without this service, would require hospital admission to an acute mental health ward, due to mental health crisis which impacts on the person's ability to cope with day-to-day activities. We work collaboratively with Psychiatrists, Nurses, Social Workers, OTs and other professionals to provide collective support and knowledge.

Main duties of the job:
- The role of a Band 5 Mental Health Practitioner within the Crisis Resolution and Home Treatment is to provide specialist acute crisis psychiatric interventions and expertise to services users in crisis 24/7 per 52 weeks per year within their own home/residence, providing a gate keeping service to the inpatient wards and community teams.
- To facilitate early discharge (Step Down) where appropriate and provide an effective viable alternative to hospital admission for clients in crisis by developing an individualised approach to the practical and functional needs of the service user.
- The successful candidate will be tasked with performing comprehensive assessments of patient nursing needs, planning, implementing and evaluating care delivery according to changing health care needs.
- Taking responsibility for the care needs of individual and groups of clients/service users.
- To collect, collate and fully accurately report patient information, maintaining accurate complete and contemporaneous nursing records in line with NMC / HCPC and Trust standards, involving clients/service users and carers / relatives in the assessment, planning, delivery and evaluation of care.
- The post holder will organise and prioritise their own work and where appropriate the work of the nursing team on a day to day basis.
- Supervising others and participating in their education and development.
- Contribute to the development of services, new ways of working and the implementation of change.
