We are seeking an experienced Heart Failure Specialist nurse to join our existing team who provide specialist nursing service across Mid Nottinghamshire. Care closer to home is becoming integral to the future of the NHS and the Community Health Services Division of Notts HC are at the forefront of this exciting journey. Working with our system partners in creating collaborative ways to improve physical health care services, activities and interventions in local communities, which meet the needs of our patients in the places they call home.
Main duties of the job:
- Support caseload management of patients with Heart Failure, providing care plans as appropriate.
- Work closely with members of the Cardio Respiratory Team and the wider MDT, including acute colleagues, GPs, and other health professionals.
- Maximise patients' quality of life and encourage self-management.
- Care takes place in community clinic settings and patients' own homes.
The service operates from 8.00-5.00pm, and it is essential that you are a UK car driver/driver licence holder and able to travel independently across the county.

Person Specification:
- Essential: V300 Non Medical Prescriber, Registered Nurse (General) with NMC pin.
- Desirable: BSc hons health related degree, a recognised Heart Failure qualification at minimum of diploma level.

Contractual Requirements:
- A full UK driving license and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
