An exciting opportunity has arisen for an experienced and motivated clinical pharmacist to join the Community Medicines Optimisation Service at Manchester University NHS Foundation Trust.

The post holder will provide specialist clinical leadership and medicines optimisation support to MFT Children's Community Services. The role will also oversee and, where required, provide clinical pharmacy support to Francis House Children's Hospice.

The Community Medicines Optimisation Service is a diverse community pharmacy team providing clinical and medicines governance support across a wide range of services. Its work includes clinical medicines optimisation, policies and standard operating procedures, medicines information, audit, guidance, prescribing monitoring and support for the safe and effective use of medicines.

We are looking for a pharmacist with substantial post-registration experience, a strong clinical pharmacy background and experience of supporting children's community services. You will need excellent clinical knowledge, sound understanding of medicines governance, and experience of developing guidelines or protocols.

Main duties of the job

• providing specialist clinical and therapeutic advice to healthcare professionals, patients and families
• supporting medicines-related aspects of admission to and discharge from Francis House
• developing and reviewing medicines policies, procedures, guidelines and standard operating procedures
• supporting the development of Patient Group Directions
• providing leadership on medicines governance, risk management and incident review
• developing and delivering education and training for healthcare professionals
• supporting audit, service evaluation and evidence-based practice
• monitoring prescribing, medicines use and expenditure
• working with multidisciplinary teams to improve the safety, quality and coordination of care
• contributing to the development of medicines optimisation services across MFT Community Services.

• Evidence of commitment to CPD
• Diploma / MSc in Clinical Pharmacy or equivalent clinical experience

Desirable

• A significant amount of post-registration experience.
• Experience of supporting community children services.
• Excellent clinical pharmacy knowledge
• Sound understanding of biochemical and haematological data
