Job summary

We are looking for an enthusiastic and compassionate Practice Nurse to join our friendly and supportive primary care team. This is an exciting opportunity for an experienced nurse who is passionate about delivering high-quality, patient-centred care and making a positive difference to the lives of our patients.

You will be an integral part of our multidisciplinary team, working alongside GPs, healthcare professionals and the wider practice team to provide a high standard of clinical care. Main duties of the job

Long-term condition management: run clinics and monitor patients with asthma, diabetes, COPD, CVD etc including reviewing patients, identifying poor control and liaising with GP colleagues to optimise health and medication.

Women's and Sexual Health: perform cervical cytology screening, provide family planning advice and pill checks

Full range of vaccinations and immunisations including childrens routine schedule, adult vaccinations including seasonal and catch up programmes

Diagnostic Tests including venepuncture, performing ECGs, BP and urinalysis

Travel Health advice and vaccinations (we are not a yellow fever centre)

Ad hoc injections including administration of Hydroxocobalamin, Zoladex, Prostap, experience administering Inclisiran

Understanding of the clinical targets required in primary care for QOF and local enhanced services is essential to maximise patient outcomes and practice achievement

Good understanding of infection prevention and control processes and best practice

Job responsibilities

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced registered Nurse and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training.

Requirements and Qualifications

  • Valid registration with the Nursing and Midwifery Council (NMC) and full nurse qualification.
  • Primary Care nursing experience
  • Strong clinical assessment and communication skills
  • Good team working skills
  • You must have full UK working eligibility
  • Offer subject to satisfactory references and enhanced DBS check

Qualifications

Essential

  • Valid registration with the Nursing and Midwifery Council (NMC) and full nurse qualification.
  • Accredited to carry out cervical screening and women's health consultations, children's and adult's vaccinations - both routine and for travel or ad hoc.
  • Familiar with managing chronic long-term conditions through annual and ad hoc reviews - giving advice and recommending further assessment by GP, Community, Specialists where indicated.
  • You must have previous experience working as a nurse in general practice
  • You must have full UK working entitlement

Desirable

  • Womens Health: experience fitting implants and pessaries would be useful though not essential
