Health Care Assistant / Nursing Associate in Primary Care Manchester

An exciting opportunity has arisen for an enthusiastic and motivated qualified Health Care Assistant / Nursing Associate to join our team. You will be joining a well-established successful nursing team of 2 ACPs, 1 trainee ACP, 2 very experienced Practice Nurses, and 1 healthcare assistant who work in conjunction with a great wider team of clinicians, a very supportive management team, as well as administrative function.

Main duties of the job:

• The applicant must be a qualified HCA/ Registered Nurse Associate on the NMC register and must have the Nursing Associate foundation degree qualification.
• The ideal candidate would work 25 hours per week ideally Wednesday, Thursday and Friday, but there is some flexibility around this.
• You will provide and maintain a high standard of nursing care for patients, as well as providing nursing assistance to the Doctors and other members of the primary healthcare team.
• The duties will include all tasks normally undertaken by a HCA/Nursing Associate working in General Practice, including health promotion and room checks (infection control duties).

We have a strong GP team which also includes 2 ACPs, 1 trainee ACP, 3 Practice nurses, a Practice Pharmacist, and a Health Care Assistant who work closely to deliver care to our 10,500+ patient population. Our premises also house Community mental health and Social prescribing clinics. We practise from purpose-built premises with easy access to Manchester city centre and the North West motorway network. As an established training practice, we are proud to have trained Advanced Nurse Practitioners, Foundation year doctors, and GPST trainees.

Job responsibilities:

• To assist and perform routine nursing tasks related to patient care, such as routine BP checks, ECG, BMI and lifestyle advice, as directed and agreed with line manager, registered general nurses and GPs.
• To support patients to improve and maintain their mental, physical, behavioural health and wellbeing by using behaviour change interventions, such as undertaking health checks (physical and mental), smoking cessation services.
• Support the Practice Nurse with the management of patients with long-term conditions/chronic diseases, using a holistic patient centred approach.
• Assist in and perform routine tasks related to patient care as directed by the Practice Nurse GP or other member of the Primary HealthCare Team.
• Be able to undertake baseline observations and identify signs of improvement, report any deteriorations or concerns, e.g. pulse, blood pressure, respirations, oxygen saturations, peak flow, spirometry, blood glucose levels, diabetic feet check, participate in screening programmes and Annual Reviews for example in patients with Learning Disabilities.
• Perform cervical smear screening on completion of additional training as guidance RCN Position Statement on Registered Nursing Associates (RNAs) training in cervical sample taking.
• Administer medications as prescribed and in accordance with NMC standards e.g. via nebuliser or injections using sub-cutaneous or intramuscular route under Patient Specific Directions.
• Following agreed protocols with referral to Practice Nurse or GPs as appropriate assist in the assessment, planning, development implementation and evaluation of programmes to promote health and wellbeing and prevent adverse effects on health and wellbeing.
• Deliver opportunistic health promotion using opportunities such as new patient medicals and NHS health checks.
• Document consultations accurately and appropriately, according to NMC guideline.
• Ensure awareness of statutory and local clinical adult and children safeguarding protection procedures, including systems of referral.
• Ability to recognise signs and symptoms of abuse/neglect.
• Ensure infection prevention and control guidelines are maintained.
• To be responsible for providing the appropriate nursing evidenced based care of an allocated group of patients.
• Be an accountable professional who is responsible for their actions, working in line with NMC standards of proficiency and recognise and work within the limits of their competency.
• To work as part of multidisciplinary team using resources efficiently.
• To deliver quality care to patients and to work effectively within the nursing team.
• To be competent in carrying out a variety of clinical procedures.

Qualifications:

• Essential: Registered Nursing Associate, Nursing Associate Foundation Degree qualification, Ability to provide quality care, Good interpersonal and communication skills, Disclosure and Barring Service Check, UK Registration.
• Desirable: Excellent IT skills.

Experience:

• Essential: Experience of working within a nursing team, Experience of working in a GP practice.
• Desirable: Primary care experience.
