Community Nurse The Concourse in London

Are you an experienced and compassionate Registered Nurse looking to develop your career in community nursing? We are looking for an enthusiastic and motivated Band 6 Senior Community Nurse to join our supportive team, providing high-quality, person‑centred care to patients with a range of complex and long‑term health needs within community settings.

You will work autonomously while being part of a wider multidisciplinary team, managing a varied caseload, completing comprehensive assessments and developing personalised care plans that promote independence, choice and self‑management. You will work closely with patients, families, carers, GPs and other health and social care professionals to coordinate care and help prevent avoidable hospital admissions. You will also have the opportunity to develop your leadership skills by supporting junior colleagues and students, contributing to service improvement and helping us deliver consistently high standards of care.

Main duties of the job

• Work autonomously to assess, plan, deliver and evaluate high‑quality, person‑centred nursing care for patients with a range of complex and long‑term health needs.
• Manage and coordinate a caseload, taking a proactive approach to supporting patients in the community and helping to prevent avoidable hospital admissions.
• Complete holistic assessments that consider patients' physical, psychological and social care needs, including the needs of their families and carers.
• Work collaboratively with GPs, community nurses, operational leads and other health and social care professionals to identify and support patients who would benefit from proactive case management.
• Advocate for patients, ensuring their individual needs, preferences and choices are at the heart of their care while promoting independence and self‑management wherever possible.
• Recognise and respond appropriately to changes in a patient's condition, maintaining accurate records and escalating concerns when required.
• Coordinate care and make appropriate referrals to other services to ensure patients and carers receive the right support at the right time.
• Provide clear information, advice and support to patients, families and carers, empowering them to make informed decisions about their health and wellbeing.

Person Specification

Education/Qualification

• Essential: RGN / RN with current registration; Evidence of recent and continuing post-registration professional education and training equivalent to post‑graduate diploma level.
• Desirable: Mentor training; Has undertaken formal training in staff and team management; Nurse prescribing qualification.

Experience

• Essential: Evidence of recent post‑registration nursing experience; Experience of negotiating and liaising within a multi‑disciplinary team; Experience of working with people with complex health needs; Experience of supervising junior staff and co‑ordinating a team on a day to day, regular basis; Experience of teaching and mentoring others including students; Experience of implementing evidence‑based care in practice; Experience of undertaking comprehensive health needs assessment and writing care plans.
• Desirable: Community nursing and/or primary care post‑registration nursing experience.

Skills and Knowledge

• Essential: Principles of care for patients with long term conditions, relevant NSFs and other national standards; NMC Code of Professional Conduct and application to practice; Personal Development Planning; Primary Health Care - structures and roles (overview); The audit process; Applying evidence based care into practice; The principles of clinical governance and application to practice; Clinical supervision - purpose and application; Managing difficult situations and conflict resolution; How equal opportunities can be implemented in practice; Has high level of competence in a range of clinical skills; Basic computer skills.
• Desirable: Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
