An exciting opportunity for a Clinical Pharmacist has arisen to join our Walnut Medical Centre Practice and based exclusively at this practice. Previous experience working on site in a general practice is essential. There may be room for negotiation on the terms and salary depending on experience. The post is fully based at Walnut Medical Centre although remote working for some/most sessions is possible. The successful applicants will work closely with the existing clinical teams to deliver patient care working innovatively and collaboratively. The number of hours is flexible between 10 hours to 15 hours approximately and actual hours can be discussed. This can be a combination of on site and remote working. This is a fantastic opportunity for the successful candidate to develop their clinical skills in a positive and excellence-focused environment. This post is offered as a permanent position. Completion of CPPE and Independent Prescriber qualifications are an advantage and would be preferred.
Main duties of the job include delivering expertise in clinical pharmacy and providing Clinical Pharmacy input to the teams based at Walnut Medical Centre. You will assist in the delivery of the practice vision and collaborate across multidisciplinary networks to promote new ways of working to deliver the best possible care to the local population. You will work as part of a multi-disciplinary team in a computer and possibly patient-facing role, supporting the Clinical team in providing pharmacist management support. You will be involved in delivering requirements of the DES and practice pharmacy requirements. You will be skilled in undertaking audits and quality improvement projects and ideally have knowledge of Eclipse.
The Clinical Pharmacist will have a key role in supporting delivery of the practice targets and PCN objectives. Key qualities will be the ability to build effective relationships with patients, GPs, nurses, community pharmacists, hospital and practice staff, and to work effectively as part of a team. Demonstrable general pharmaceutical skills and knowledge in core areas, including long term conditions, together with an understanding of general practice are essential. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
Job responsibilities include:
- Patient facing long-term condition clinics: See patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma).
- Review the ongoing need for each medicine, monitoring needs, and support patients with their medicines taking ensuring they get the best use of their medicines.
- Make appropriate recommendations to GPs for medicine improvement.
- Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring.
- Work with care home staff to improve safety of medicines ordering and administration.
- Provide patient facing clinics for those with questions, queries and concerns about their medicines in the practice.
- Provide a telephone help line for patients with questions, queries and concerns about their medicines.
- Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
- To reconcile medicines following discharge from hospitals, intermediate care and into care homes.
- Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care.
- Produce and implement a practice repeat prescribing policy.
- Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches.
- Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
- Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.
- Support public health campaigns.
Person Specification Qualifications:
- Must have experience of working within general practice.
- Degree in Pharmacy, MPharm/BPharm.
- GPhC Registered.
- Full driving licence.
- CPPE IP Qualified or working toward an IP qualification.
- General Practice and understanding of medicines optimisation roles.
Desired experience includes being a member of the RPS, holding a Clinical Diploma, and being clinically systems trained (SystemOne/Emis/Vision).
