Senior Registered Nurse - REACT Ipswich

An exciting opportunity has risen for enthusiastic and motivated individuals to join our Reactive Emergency Assessment Community Team (REACT) as a Senior Registered Nurse. The REACT team is a multi-disciplinary integrated community service within ESNEFT incorporating OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients. Our team delivers excellent patient focused care to patients in the East Suffolk community.

The role involves travelling throughout East Suffolk delivering acute care in patients' homes as a part of the REACT nursing team, both during core hours and out of hours. The post holder will be working in an internal rotation. The successful candidate will be expected to work a mixture of shifts that include the following: 11-11 triage shifts, early (0800-1600), late (1500-2300), twilight (1800-2300) and night shifts (2230-0830) including weekends and bank holidays as the service requires. During out of hours between 18:00-8:00 the Nurse will be accompanied with a Health Care Assistant. Pool cars are accessible for staff to use and it is essential that the post holder has a valid driving licence.

Main duties of the job

In this role, during the core hours, the team's main responsibilities include working in partnership with the OPAT (Outpatient Parental Antibiotic Therapy) to administer intravenous antibiotics via PICC line or cannula for patients receiving antibiotics in the community. Core hours will also include assessment and delivery of care to patients in crisis to allow admission prevention. During the out hours, main responsibilities include routine and emergency response to wound care, catheter care for all types of catheters, diabetic care, End of Life care, Anticoagulant medication, Pressure Area care, Venepuncture and Sub cut Fluids in order to ensure 24 hours Nursing care cover in the community. You would be liaising with District nursing service to ensure continuity of care and other community services where needed. The role will also include being the shifts team lead, triaging and co‑ordinating case load, scheduling visits, carrying out appraisals and clinical supervision for more junior staff. Part of this role is also to support senior management in the daily running of the team and promoting change.

Person Specification

• Experience
• Essential: Experience leading teams, Clinical expertise, Coaching/Mentoring, Managing change, Evidence of managing and improving the patient experience, Previous acute, primary care or community experience, Triage experience.
• Desirable: Previous experience at Band 6, Evidence of involvement in research and audit, Evidence of teaching/assessing, Interviewing recruitment skills.
• Qualifications
• Essential: Registered Nurse, First Level, Post registration qualification relevant to area up to post graduate diploma level.
• Desirable: Nursing / Management degree, Willingness to undertake further academic study, NVQ assessors qualification, Leadership modules/course undertaken.
• Knowledge
• Essential: Sound evidence of clinical ability, Articulates understanding role of Band 6, Prioritising clinical issues, & managing clinical risk, Provides evidence of ability to make changes in nursing practice, Ability to recognise a deteriorating patient and accelerate the patient to the relevant professionals/agencies/services in a timely manner, Good knowledge and understanding of diabetes management, Good knowledge and understanding of End of Life Care/management.
• Desirable: Sickness absence management, Rota management, Knowledge of the different teams/services within REACT, Knowledge and understanding of the 2 hour urgent community response (UCR), Knowledge of community databases such as EPIC, Able to request and translate blood results, Ability to undertake audits.
