Advanced Clinical Practitioner (ACP) - Community Emergency Medicine Horncastle

You will manage a wide range of undifferentiated presentations, support virtual wards, utilise community pathways and help patients avoid unnecessary hospital attendance or admission wherever it is safe to do so. Working across pre-hospital, urgent and community care environments, you will make a tangible difference to patient outcomes while supporting prevention, early intervention and care closer to home.

Main duties of the job

• The Community Emergency Medicine Advanced Clinical Practitioner (ACP) delivers advanced assessment, diagnosis, treatment and clinical decision-making for patients in community and pre-hospital settings across Lincolnshire.
• Working autonomously and within a multidisciplinary team, the ACP manages a wide range of undifferentiated presentations, supports virtual wards and utilises community pathways to provide safe, effective care closer to home.
• The role focuses on preventing avoidable hospital attendance and admission through holistic assessment of patients' medical, functional and social needs.
• The postholder will identify risks, support early intervention and safeguarding, make referrals to appropriate services, and empower patients and carers to make informed decisions about their care.
• The ACP will operate as part of a mobile clinical team, responding to incidents across the county, undertaking dynamic risk assessments and maintaining accurate clinical records.
• They will work collaboratively with urgent and emergency care partners, primary care, social care and specialist services, while contributing to clinical governance, service improvement, education and mentorship.
• As a member of LIVES, the postholder will also support the charity's mission through community engagement, awareness raising and fundraising activities.

Main Responsibilities

• Respond to patients in community settings, providing assessment, diagnosis, and treatment for a wide range of clinical presentations.
• Provide clinical support to virtual wards.
• Deliver care aimed at avoiding unnecessary hospital attendance or admission, supporting safe management within the community wherever appropriate.
• Undertake holistic patient assessment, considering medical, functional, and social needs to inform decision-making.
• Promote prevention and early intervention by identifying deterioration risks, safeguarding concerns, and unmet health or social care needs.
• Utilise and develop referral pathways into community, primary care, urgent care, and specialist services.
• Support patients, families, and carers to make informed decisions about care, promoting independence and self-management where appropriate.
• Work within agreed clinical guidelines, scope of practice, and organisational policies, seeking advice or escalation where required.
• Provide clinical support and advice to colleagues, including remote consultation where appropriate.
• Maintain accurate, contemporaneous clinical records and documentation in line with governance and information standards.
• Work collaboratively with UEC partners, primary care providers, social care teams, and wider system partners.
• Participate in clinical governance activities, including audit, incident reporting, and service improvement.
• Support training, mentorship, and development of colleagues within the multidisciplinary team.
• Actively support the organisations charitable mission by contributing to fundraising, awareness, and engagement activities alongside clinical duties.

Operational Duties

• Operate as part of a mobile clinical team responding to incidents across Lincolnshire.
• Work in partnership with operational control and dispatch teams to ensure appropriate patient allocation and response.
• Conduct dynamic risk assessments in community environments to ensure safety of patients, self, and others.
• Maintain readiness of vehicles, equipment, and clinical resources in line with organisational policies.
• Liaise with operational managers and clinical leadership teams as required.

Person Specification

Knowledge and skills

• Essential: Ability to work autonomously and make complex clinical decisions.
• Strong understanding of risk management in community environments.
• Excellent communication and interpersonal skills.
• Understanding of prevention, admission avoidance, and system working.
• Desirable: Knowledge of local health and social care systems.

Experience

• Essential: Significant experience managing undifferentiated presentations in adults and children in community, urgent, or emergency care settings.
• Desirable: Experience in primary care, urgent care, acute medicine, emergency medicine and/or pre-hospital care.
• Experience working across organisational boundaries and community pathways.
• Experience in teaching, supervision, or mentorship.

Qualifications

• Essential: Registered healthcare professional (NMC, HCPC or equivalent) with no restrictions on practice.
• MSc in Advanced Clinical Practice (or equivalent).
• Independent Prescriber qualification (V300 or equivalent).
• Immediate Life Support training or higher.
• Full UK driving licence.
• Willingness to undertake enhanced DBS clearance.
• Desirable: RCEM or other similar ACP credentialling.
• Diploma/Fellowship in Immediate Medical Care.
• Diploma in Urgent Care.
• Advanced Life Support Provider.
