We are pleased to announce an exciting opportunity for a Band 6 Sister/Charge Nurse within our Frailty Pathway. While the base post is on Ward 12 as Ward Sister, this role sits within the wider frailty pathway, meaning there may be opportunities to work across different areas within the pathway. As a key member of the team, you will play a vital role in delivering high-quality care to patients requiring support for frailty-related conditions. This is an excellent opportunity for an experienced, motivated, and enthusiastic nurse with a strong interest in caring for patients with frailty needs. The Frailty Pathway includes AFU, Ward 12, and Sunderland Ward. There will be opportunities to rotate across these areas for defined periods, supporting both professional development and service delivery. Ward 12 is a 24-bedded ward with a friendly and supportive team, committed to delivering high-quality nursing care.
Main duties of the job:
- Promote a ward culture that prioritises the prevention of deconditioning by embedding meaningful, person-centred activities into daily care.
- Uphold the Health Board's values of respect, fairness, integrity, honesty, openness, kindness, compassion, and care in all interactions, and actively encourage these behaviours within the team.
- Deliver compassionate, high-quality, and effective patient-centred nursing care, alongside fair, transparent, and accountable people management practices.
- Provide visible and professional nursing leadership in line with NMC standards, offering expert clinical advice, support, supervision, and guidance.
- Ensure all care delivery and management processes comply with legal, professional, and organisational requirements, and are grounded in evidence-based practice.
- Foster a positive learning environment that supports the continuous professional development of nursing staff, students, and the wider multidisciplinary team, enabling them to deliver safe, effective care that maintains patient independence and wellbeing.

Person Specification:
Qualifications and Knowledge:
- Essential: Registered Nurse (Part 1), Current NMC Registration, Bachelor degree (or equivalent experience), Pertinent postgraduate diploma (or equivalent development / experience and a willingness to undertake appropriate post graduate diploma level study), Bronze IQT.
- Desirable: Teaching qualification (or equivalent experience), Recognised management qualification.
Experience:
- Essential: Appropriate experience within specialty and able to demonstrate sound knowledge of relevant specialty, Able to demonstrate sound knowledge of NMC Code (2015), Experience of working in a multi-professional environment, Experience of running a ward/area/department, Experience of working with pre-registration nursing / midwifery students, Experience of formal /informal teaching and of mentoring pre-registration nursing or midwifery students, Understanding of wider nursing and health care related agenda, IT skills, Supervisory management skills, Able to relate theory into practice through reflective skills.
- Desirable: Experience of coordinating a team, Experience of facilitating learning in and from practice, Previous experience of leadership and management, Awareness of current issues in nursing and specialty, Awareness of financial and resource management.
