Advanced Clinical Practitioner - Cardiology Halifax

Calderdale and Huddersfield NHS Foundation Trust are currently looking for an experienced and enthusiastic part-time Advanced Clinical Practitioner (ACP) to join our Cardiology team. You will be the second ACP within the service and will deliver independent rapid access chest pain clinics as well as other clinical and non-clinical duties:

• Teaching/Training opportunities available
• ACP forums and dedicated supervision to support the delivery of the Trust wide ACP strategy
• ACP celebration events and opportunities to attend conferences
• Allocated Medical Supervision

Main duties of the job:

• Assess individuals, families and carers holistically.
• Communicate effectively with patients' carers and the multidisciplinary team.
• Use their expert knowledge and clinical judgement to decide whether to refer patients for investigations and make diagnoses.
• Decide on and carry out treatment, which may include the prescribing of medicines, or referring patients to an appropriate specialist.
• Ensure the provision of continuity of care.
• Assess and evaluate, with patients and/or carers, the effectiveness of the treatment and care provided and make changes as needed.
• Work autonomously within a wider health care team and as a lone practitioner.

Job responsibilities:

• To work closely with the Cardiology medical and nursing teams to provide effective care to our Cardiology patients in both outpatient and inpatient settings.
• Practice as an autonomous professional in a self-directed manner, with confidence, authority and expertise, undertaking a comprehensive range of clinical procedures.
• Have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made.
• Work clinically to optimise health and wellbeing, through comprehensive history taking and physical assessment of patients.
• Drawing on a diverse range of knowledge and skill, determine appropriate therapeutic interventions.
• Plan, prioritise and manage complete episodes of care, for patients presenting with undifferentiated and undiagnosed conditions.

Person Specification:

Qualifications/Training

• Essential: Registered general nurse or Allied health care professional, Teaching/Assessment, Mentoring qualification, Previous experience of formal/informal teaching of patients and staff.
• Desirable: Masters in Advanced Practice or equivalent, Non Medical Prescriber.

Knowledge, experience & expertise

• Essential: Experience in working as an autonomous practitioner within a specialty, Knowledge of the key professional issues and NMC/AHP guidelines related to professional practice, Evidence of being able to work across organisational boundaries, Experience of applying research evidence to clinical practice, Evidence of continuing professional development, Commitment to maintain own fitness for practice through maintaining a personal profile of competencies.
• Desirable: Working knowledge and clinical management of patients specific to role, Demonstrates up to date knowledge and expertise of Advanced Practice.

Communication & relationships

• Essential: Demonstrate a clear vision of the role and service, Ability to initiate sustain and evaluate change, Demonstrates skilled written and verbal communication, presentation and decision making skills.
