Mental Health Practitioner Referral and Assessment Hub Eccles

As part of community transformation, we are changing the way we manage referrals. We have developed a referral and assessment hub and are looking to recruit B6 Mental Health Practitioner to support management of referrals and assessments within the hub.

You will support the triage completed by the senior practitioner and will jointly support the decision making on the onward journey for that service user. You will be aligned with one of our CMHT's but may work across all three to complete assessments. You will provide duty assessments for the referral and assessment hub at the designated base or at a venue agreed with the service user and carer. You will need to be able to travel citywide across the Salford Footprint. There may also be times when that duty assessment will require you to inreach into the ward if they are an inpatient at the time of referral.

This post is a full time position (37.5 hours per week) and the working hours will be 9am - 5pm, 5 days per week. The post is based at Cromwell House. The post holder will be expected to travel across the Borough to attend satellite clinics across Salford.

• Triage of referrals coming into the referral and assessment hub as part of a team.
• Providing an onward journey if not appropriate for Community Services.
• Completing assessments for those people requiring specialist mental health services under Community Mental Health Team.
• Complete assessment for those inpatients who may require a secondary service upon discharge.
• To link in with the MDT and attend a weekly meeting to discuss your assessments.
• To promote awareness of the role of the referral and assessment hub, within local service area/team, GP practices and the wider community.
• Supporting service user, carer and stakeholder involvement.
• Promote a culture of learning, education and professional development.
• Championing quality assurance by establishing quality standards relating to the service user and carer experience, implementing Divisional Action Plans and undertaking complaint investigations.
• Adhere to the principles of safeguarding of children and vulnerable adults in line with Trust Policy and local reporting procedure and ensuring junior staff compliance with this.

• Essential: Post registration experience of working in multi-disciplinary, Community Mental Health Team, Home Treatment team or other mental health community services.
• Evidence of post registration training and development in assessment and formulation.
• Knowledge and understanding of the needs of carers, service users and the wider community.
• Desirable: Experience of participating in audit and an understanding of implementing Action plans relating to the outcome of audits.
• Previous experience in a clinical leadership role.

• Essential: Professional qualification and current registration (Qualified RMN, OT or Social Worker).
• Desirable: Post registration training relevant to mental health/leadership/management.

• Essential: Offer advice and consultation to members of the multi-disciplinary team based on solid clinical experience.
• Facilitate a culture of lifelong learning, including involvement in the process of Individual Development and Personal Review (IDPR), and Personal Development Plans.
• Participate in and ensure provision of Clinical Supervision, both as Supervisee and Supervisor and an understanding of the Knowledge, Skills Framework under Agenda for Change.
• Promote a learning environment for students.
• Ensure compliance with policy, procedure, and protocol, in consultation with the Team Managers, other Senior Practitioners and Professional Leads.
• Participate in multi-agency meetings e.g., Protection of Vulnerable Adults, Child Protection.
• Understand the Clinical Governance agenda and provide professional representation in local groups supporting consultation and implementation.
• Full understanding of the Care Programme Approach including the needs and rights of Carers.
• Understanding of current national policy and initiatives in relation to severe and enduring mental health issues.
• Good knowledge of assessment and triage of referrals.
• Desirable: Support the Team Managers and other Senior Practitioners / Professional Leads in the daily operation of the Teams.
