Practice Nurse Croydon

We are seeking an experienced and motivated Practice Nurse to join our clinical team at St James Medical Centre. This role offers an opportunity for a skilled nurse to work within a supportive general practice environment, delivering a broad range of nursing services to our registered patient population across both of our practices in Croydon and New Addington. The successful candidate will work alongside GPs and the wider practice team to provide planned and preventative care, support patients with long-term conditions and contribute to improving health outcomes within our community. This is a varied role requiring clinical expertise, good judgement and the ability to work independently while maintaining effective communication with colleagues and patients. The ideal candidate will be committed to delivering excellent standards of nursing care and supporting the continued development of primary care services. Main duties of the job include:

• Providing safe, effective, and patient-centred nursing care in a primary care setting
• Undertaking long-term condition management clinics including diabetes, asthma, COPD, hypertension, and cardiovascular disease
• Performing cervical screening in line with the NHS Cervical Screening Programme
• Administering childhood and adult immunisations and vaccinations
• Carrying out wound care, dressings, and minor injury management
• Supporting health promotion and disease prevention activities, including lifestyle advice and patient education
• Identifying and escalating safeguarding concerns in line with practice policies
• Maintaining accurate, contemporaneous clinical records using EMIS Web

Professional Responsibilities include:

• Working within the NMC Code of Professional Conduct at all times
• Maintaining up-to-date professional registration, revalidation, and CPD
• Participating in clinical supervision, appraisal, and personal development planning
• Actively engaging in training, reflective practice, and continuous learning
• Supporting students, new starters, and colleagues as appropriate

Quality, Governance, and Safety responsibilities include:

• Contributing to QOF, enhanced services, and local incentive schemes
• Participating in clinical audit, quality improvement, and service evaluation
• Maintaining high standards of infection prevention and control
• Complying with all practice policies, procedures, and clinical governance frameworks
• Supporting CQC standards, including Safe, Effective, and Well-Led domains

Team Working and Communication:

• Working collaboratively with GPs, pharmacists and the wider PCN team
• Communicating effectively with patients, carers, and colleagues
• Contributing positively to a supportive and respectful team culture
• Using digital systems and data to support clinical decision-making and service improvement

Technology and Innovation:

• Using clinical systems (EMIS Web, Accurx, NHS systems) effectively and accurately
• Engaging with digital tools and new ways of working to improve efficiency and patient experience
• Supporting data-driven approaches to long-term condition management and recall systems

Confidentiality and Information Governance:

• Maintaining patient confidentiality at all times in line with GDPR and NHS guidance
• Ensuring information is recorded, stored, and shared appropriately

Other Responsibilities:

• Undertaking any other reasonable duties appropriate to the role
• Flexibility to support service delivery in line with practice needs

Health & Safety:

• Taking reasonable care for own health and safety and that of others
• Reporting incidents, near misses, and concerns in line with practice policy

Person Specification:

• Registered Nurse (Adult) with current NMC registration
• Evidence of ongoing Continuing Professional Development (CPD)

Desirable experience includes:

• Previous experience working in an NHS GP practice
• Experience supporting QOF, enhanced services, or local incentive schemes
• Experience working within a multidisciplinary or PCN team
• Experience contributing to clinical audits or quality improvement projects
• Experience using EMIS Web and AccuRx
• Sound knowledge of primary care nursing and NHS clinical pathways
• Ability to deliver evidence-based care in line with national and local guidelines
• Understanding of infection prevention and control in a GP surgery setting
• Awareness of safeguarding responsibilities for adults and children
