The community palliative care team are looking for an enthusiastic, motivated registered nurse to join Thundersley/Benfleet palliative care team as a band 6.

The post holder will hold a caseload of end of Life patients and work as a key member of the Palliative Care Team delivering care alongside nurses and therapists to end of life patients.

The post holder will work to reduce all inappropriate admissions to secondary care through the use of advanced care planning and practical high quality end of life care.

Main duties of the job

To become part of the community palliative care team to provide holistic care for patients with life limiting conditions.

Person Specification

Essential

  • Enthusiasm for the discipline
