Community Nursing Associate Attleborough

Our Wymondham and Attleborough community nursing teams have a rare opportunity for a registered Community Nursing Associate. The Ketts Community Nursing Team is part of Norfolk Community Health and Care NHS Trust and part of the South Place working within and surrounding villages. The job predominantly requires visiting patients within their homes and care homes.

The job requires you to contribute to the delivery of care packages for patients with long term conditions, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible. To work within the integrated team to facilitate early discharge from hospital. To work within the integrated team to prevent unnecessary admission to hospital. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

The successful applicant will have a sound clinical background with a high level of skills and knowledge.

Main duties of the job:

• Prepare for, carry out and monitor assessments and treatments in specified clinical areas, and discharge in line with predetermined department protocols.
• Apply competency based treatment techniques/care to patients with specific conditions under the supervision of a qualified practitioner, following a prescribed treatment/care plan.
• Plan and prioritise own visits to delegated patients, and ensure these are documented in personal work diary.
• Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis.
• Be required to use tact and persuasive skills in order to gain the patient's co-operation in their treatment/care/management plan.
• Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.

Maintain appropriate patient and clinical records in line with the Trust's policies and procedures, and in line with the agreed service specification. In line with the annual personal development plan the post holder will be expected to undertake any training or development required to fulfil their role. Ensure that all patients, clients and colleagues are treated at all times in an equitable manner, respecting diversity and showing an understanding of diversity in the workplace. Job responsibilities:

The post holder will be responsible for their own daily caseload with supervision and support from the Clinical Lead and the Band 5 & 6 Nurses. You will be expected to have excellent communication skills & IT skills and be able to provide evidence of working successfully at Band 3 and above. The post holder will need to show their experience of contributing to assessments and care planning and carrying out interventions to contribute to patient health and wellbeing. The post holder will have access to training and supervision that will build upon their skills and will help them to progress as a competent band 4. The post holder will need to be a car driver with a full current driving licence.

Person Specification:

• Qualifications: Foundation degree programme or equivalent demonstrable knowledge and skills acquired. Registered Nursing associate on the NMC register.
• Experience: Experience of working within a health / care setting. Desirable: Experience of working in teams under appropriate supervision as part of the multi-disciplinary team.
• Communication: Able to communicate effectively using the English language in both verbal and written forms. Courteous, respectful and helpful at all times. Ability to communicate with members of the public and health and care providers.
• Skills, Abilities and Knowledge: Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team. Insight into how to evaluate own strengths and development needs, seeking advice where appropriate. Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role may contribute to service development. Evidence of time management skills and ability to prioritise. Intermediate IT skills. Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact. Ability to move between sites working across health and social care as required by the needs of the service. Ability to work on own initiative. Ability to take part in reflective practice and clinical supervision activities. Knowledge of when to seek advice and refer to a registered care professional. Understanding of the importance of the promotion of health and wellbeing (Making Every Contact Count).
