An NMC registered Band 5 Nurse is needed to join the Home First Team (Discharge to Assess Pathway 1) across South Staffordshire. You will assess, plan, implement and evaluate care in patients' homes, supporting safe hospital discharges and empowering independence.
You will work in a multi-disciplinary team, cover a 24/7 shift pattern, travel across a large geographic area and provide education to patients and carers while upholding professional standards and safeguarding guidelines.
