Job summary

We are expanding our City Community Mental Health Team and are seeking motivated and compassionate Band 6 Community Care Coordinators to join our Community Mental Health Team in Bradford. This is an exciting opportunity to make a real difference in the lives of service users by delivering high-quality, person-centered mental health care to the people of Bradford. As a Care Coordinator, you will manage a defined caseload, working autonomously and collaboratively within a multidisciplinary team. You will be responsible for assessment, planning, intervention, and evaluation of care, ensuring that service users receive safe, effective, and recovery-focused support.

Main duties of the job

Key Responsibilities:

• Coordinate and deliver community mental health care in line with Trust policies.
• Undertake comprehensive assessments and manage risk effectively.
• Work collaboratively with service users, carers, and partner agencies.
• Administer and monitor medication in accordance with NMC and Trust guidelines.
• Participate in case conferences and contribute to service development.
• Promote equality, diversity, and safeguarding in all aspects of care.

Person Specification

Qualifications

Essential

• Professional qualification in mental health nursing, social work, occupational therapy
• Registration with Professional Body (e.g. NMC, SWE, BAOT)

Experience

Essential

• Minimum of 6 months post basic experience in Mental Health
• Experience of working in a community mental health setting.

Desirable

• Experience of being clinical adviser to junior staff or students.
