HIV Clinical Nurse Specialist Bradford

The post holder will be an independent expert practitioner and will carry continuing responsibility for a defined caseload of HIV positive clients. This includes the setting of standards of care, the assessment of client needs, and the development, implementation and evaluation of a holistic program of health care which is evidence based. The post holder will be responsible for the delivery of advanced nursing skills within complex patient care pathways and will help lead the department in the development of clinical care systems, protocols and guidelines. This post will be responsible for providing and developing excellence in HIV clinical practice and be responsible for the teaching, mentoring and assessment of students and staff within the department. The post holder will ideally be a non-medical prescriber.

They will work collaboratively with teams both within the health systems, and the community in order to provide high quality seamless service to clients/patients both in the hospital and community setting.

• Deliver expert clinical care to patients living with HIV across outpatient and community settings.
• Undertake comprehensive patient assessments, including physical, psychological and social needs.
• Manage a defined caseload including complex cases.
• Lead and independently run nurse-led clinics, including monitoring, treatment support and routine follow up care.
• Provide counselling, education and adherence support.
