Joining us as a Band 6 Community Psychiatric Nurse, you will be part of a large multi-disciplinary service with over 80 staff supporting the transition of patient care from secure settings into the community, which feeds into various sites including Reaside, Ardenleigh, Tamarind, St Andrews and Hillis Lodge (Low Secure) and Elliot House (Approved Premises). We offer extensive support to staff in their role as Care Coordinator, which includes supervision, reflective practice, art therapy, autism support and case-busting sessions. We also provide flexible working patterns, including working from home when needed, and pool cars are available for community visits.

FIRST is a standalone service providing our own home treatment and crisis community support to our service users; our CPNs and Social Workers are on a rota to jointly provide cover for on-call shifts (approximately 4 times per month) with support from On Call Responsible Clinician during evenings, weekends and bank holidays so that we can respond to urgent situations by telephone and complete home visits in emergencies (standby on-call payments and overtime are payable).

Main duties of the job:

• To undertake the duties of care coordinator in the assessment, planning, implementation and evaluation of the care and treatment needs for a defined group of service users accessing Forensic Intensive Recovery Support Team (FIRST).
• To provide professional advice and support to other stakeholders involved in service users' care which may include families, carers and care agencies, accommodation and housing providers, probation and criminal justice colleagues.
• To undertake a multi-disciplinary approach to care and work collaboratively with members of the team which may include Responsible Clinician, Social Worker, Occupational Therapist, Psychologist, Support Time And Recovery Worker (STAR), Specialist Substance Misuse Workers, and Peer Mentors.
• To support colleagues when required with day-to-day tasks and act as a role model and resource to Band 5 CPNs in the team and student nurses.
• To advocate for the client group we work with whilst promoting a culture of recovery and optimism.

Training & Qualifications:

• Essential: Registered RMN and NMC registration
• Desirable: Student Mentor Qualification & Preceptor Training.

Knowledge & Experience:

• Essential: Substantial post-registration experience in mental health settings. Understanding by post-holder of their responsibilities under MHA and CPA, Clinical Governance, Risk Assessment, Safeguarding, Child Protection Legislation and NICE guidelines.
• Desirable: Experience of working within a Forensic Mental Health, Prison or Probation Setting. Experience of working within a Community Mental Health Team.

Skills:

• Essential: Comprehensive Assessment, Risk Management Skills & Time Management
• Desirable: Training in Phlebotomy & Psycho-Social Interventions

Other:

• Essential: Genuine interest in working with the client group and independently mobile with the ability to travel for regional coverage.
