Are you an experienced Physiotherapist looking to take the next step in your career? We are seeking a passionate and motivated B5 Physiotherapist to join our dynamic team and play a key role in delivering high-quality, patient-centred care.
The Integrated Neighbourhood team is a community-based service which supports and works with people, either in their own home or in Residential or Nursing Homes.
The aim of the service is to maximise and maintain independence based on patients identified needs through rehabilitation. Following an initial assessment and, in consultation with the service user, a care plan and rehabilitation programme is drawn up.
As our services transform, there may be times you will be expected to support both nursing and therapy in the delivery of care, within an appropriate scope of practice and following relevant on-the-job training.
Main Responsibilities
- To carry out therapeutic assessment, diagnosis and treatment with patients using clinical reasoning person centred goals, who may have diverse presentations and a range of physical and psychological conditions within the context of a community-based, multi-disciplinary team.
- Responsibility for up-to-date record keeping within professional guidelines and HCRG Care group Record keeping policy.
