Help shape a new approach to community mental health care across Buckinghamshire!

We’re establishing a new Intensive Case Management (ICM) and Rehabilitation Pathway, bringing together specialist professionals to deliver flexible, proactive support for adults with some of the most complex mental health needs.

We’re looking for a Mental Health Nurses who are committed to building therapeutic relationships, working collaboratively across services and delivering recovery-focused care in the community. As part of this newly established pathway, you’ll play an important role in developing a service designed around continuity, assertive outreach and person-centred support.

This is a community-based role where you’ll meet people where they are, whether that’s at home, in community settings or Trust premises. Rather than relying on traditional clinic appointments, you’ll work proactively to engage people who may struggle to access or remain connected with services, helping to reduce relapse, prevent crisis and improve long-term outcomes.

Main duties of the job

As a Community Mental Health Nurse, you’ll manage a defined caseload of adults with complex mental health needs, providing specialist nursing assessments, evidence-based interventions and personalised care planning.

Working autonomously and as part of the multidisciplinary team, you’ll build strong therapeutic relationships with service users and their families, contribute specialist nursing expertise to clinical decision-making, and support people to achieve their recovery goals.

You’ll also contribute to service development, quality improvement and the supervision of students and junior colleagues, helping to embed a culture of learning and continuous improvement within the pathway.

Job responsibilities

You’ll be joining a newly established, fully integrated multidisciplinary team that is committed to delivering innovative, person-centred mental health care across Buckinghamshire. Working alongside Occupational Therapists, Nurses, Consultant Psychiatrists, Psychologists, Support Workers and Clinical Leads, you’ll be part of a collaborative team where every profession plays a key role in supporting recovery.

Our Intensive Case Management and Rehabilitation Pathway has been designed to provide flexible, assertive outreach for adults and older adults with complex mental health needs who may struggle to engage with traditional services. Rather than expecting people to come to us, we meet them where they are – building trusting relationships and providing support in their homes, local communities and other settings where they feel comfortable.

As a team, we share responsibility for supporting our service users through regular multidisciplinary discussions, collaborative decision-making and a strong focus on continuity of care. You’ll have the opportunity to work creatively alongside colleagues from a range of professional backgrounds, developing holistic interventions that address mental and physical health, social needs and recovery goals together.

Person Specification

Clinical Knowledge & Experience

Essential

• Demonstrates significant post-registration experience as an RMN, with experience assessing, planning and delivering care for adults with complex mental health needs in a community or equivalent setting.

Desirable

• Experience working within a Community Mental Health Team, Rehabilitation or Intensive Case Management service.

Collaborative Working

Essential

• Experience of working effectively within a multidisciplinary team and with partner agencies to deliver coordinated, holistic care.

Risk Management & Clinical Decision Making

Essential

• Evidence of assessing and managing complex clinical risk, using professional judgement to deliver safe, effective and recovery-focused care.

Engagement & Person-Centred Practice

Essential

• Demonstrates the ability to build therapeutic relationships and engage individuals who may find it difficult to access or remain engaged with services.

Professional Development & Leadership

Essential

• Evidence of supporting colleagues, students or service development, alongside a commitment to continuing professional development and reflective practice.

Desirable

• Experience of participating in quality improvement, audit or service development initiatives.
