IC24 is recruiting an Advanced Clinical Practitioner (ACP) for the East Sussex Home Visiting Service. You will work autonomously in patients' homes, delivering advanced clinical assessments and treatment, prescribing as appropriate, and supporting urgent and primary care pathways.
You will collaborate with GPs, nurses and NHS services to ensure coordinated care, reduce unnecessary hospital visits, and drive service improvement through audits and CPD.
