An Exciting Opportunity: Advanced Nurse Practitioner/Advanced Clinical Practitioner or Advanced Paramedic Practitioner at Drybrook Surgery (25 to 30 hours)

Drybrook Surgery is seeking a dynamic, motivated, and adaptable team player to join our practice as an Advanced Practitioner or Paramedic.

The ideal candidate will be a registered nurse or paramedic with a non-medical prescribing qualification, a strong clinical background, and experience working in primary care or a similar autonomous environment.

This position is subject to an enhanced DBS check.

Main duties of the job

Our Clinical Practitioners provide care through the assessment, diagnosis, treatment, and discharge or referral of patients presenting with undifferentiated and undiagnosed injury/illness, or acute exacerbations of existing conditions.

You will assist in delivering an integrated approach to patient care across health and social care systems, working closely with practice partners and external stakeholders.

The key elements of the role include, but are not limited to

• Working as an autonomous clinician within your competencies, providing clinical assessment, diagnosis, and treatment for adult and paediatric patients during face-to-face and remote consultations.
• Accurately triaging and prioritising patient needs, utilising clinical history-taking and assessment skills to make safe, timely triage and care decisions.
• Undertaking home visits to patients who are unable to travel to the surgery.
• Referring patients to alternative care settings and secondary care where appropriate, following discussion with a GP when indicated.
• Supporting the triage of calls during core surgery hours.
• Working collaboratively within our clinical and administrative team, which includes our GP Partner, ANP Partner, Practice Nurses, HCAs, clinical pharmacist, reception, and administrative staff, alongside the wider Forest Green PCN team.

We are proud to be the first practice in the county run jointly by a Partner GP (Dr Anna Raymond) and a Partner ANP (Katie Dyer), who have successfully cultivated a highly efficient, compassionate, and holistic environment for both staff and our 4,250+ patients.

We operate from our main site on Drybrook Road, supported by a committed team of nurses, cinical pharmaist, administrative professionals, and are active members of the Forest Green PCN.

• Job responsibilities
• Clinical Practice
• Work alongside our GP and ANP partners to manage patients presenting with urgent, on-the-day assessment needs.
• Deliver coordinated care to "at-risk" patients to avoid unnecessary hospital admissions and ensure they receive treatment in the most appropriate setting.
• Coordinate clinical care through surgery consultations, clinical triage, and home visits, including proactively supporting patients discharged from acute care.
• Act as an autonomous clinician, ensuring practice remains safe, effective, and strictly within your clinical boundaries of competence.
• Examine and assess patient needs from a holistic perspective (physical and psychological), proposing evidence-based care plans.
• Prescribe safely and competently in accordance with non-medical prescribing standards, verifying the appropriateness of medicines and reviewing clinical indications.
• Support chronic disease management (such as Diabetes, Hypertension, and Asthma) and participate in clinical audits to monitor and improve the quality of care.
• Liaising with Patients, Families, and Carers
• Act as an information resource and focal point of communication for patients, carers, and families regarding their clinical management and care plans.
• Direct patients and families to external agencies, the voluntary sector, or Social Prescribing services where appropriate.
• Interfacing with Other Agencies
• Maintain a strong working interface between Drybrook Surgery, the District Nursing Team, community services, and other stakeholders.
• Participate in multi-disciplinary team meetings to optimize patient outcomes.
• Clinical Governance
• Actively participate in clinical governance processes, including clinical audit, risk management, incident reporting, and CQC compliance.
• Promptly report clinical and non-clinical safety incidents to the Practice Manager and Partners, contributing to shared learning.
• Understand, identify, and appropriately elevate any safeguarding concerns for children and vulnerable adults.
• Adhere to safe systems of work, infection control, and health and safety policies.
• Managing and Using Information
• Maintain accurate, legible, and contemporaneous clinical records on the practice's IT systems in accordance with professional codes of conduct and practice policy.
• Maintain clinical databases of at-risk patients to support service development and contractual compliance.

• A uniform will be required.
• The role is based at Drybrook Surgery but will involve regular travel across the local area for home visits.

Person Specification

Clinical Knowledge and Skills

Essential

• Strong clinical assessment, diagnostic, and treatment skills.
• Ability to record precise and high-quality clinical notes.
• ECG interpretation and venepuncture.
• Minor injury and minor illness management.
• Chronic disease monitoring (e. g., Asthma, COPD, Hypertension, Diabetes).
• Understanding of when to elevate clinical scenarios and refer to GPs.

Qualifications

Essential

• Nurses: Current unrestricted registration with the Nursing and Midwifery Council - NMC.
• Paramedics: Current unrestricted registration with the Health and Care Professions Council -HCPC.
• Masters certificate, diploma, or full Masters in Advanced Clinical Practice, a Digital Badge, RCN credentialing or equivalent advanced paramedic credentialing.
• Qualified Independent Prescriber - V300 for nurses or equivalent for paramedics registered with the appropriate professional body.
• Minor illness qualification.
• Meets professional revalidation requirements.

Desirable

• Qualified triage nurse or experienced triage practitioner.

Experience

Essential

• Experience practicing across the four pillars of advanced practice.
• Experience of working in a primary care, community, or urgent care environment.
• Experience in prescribing and conducting structured medication reviews.
• Experience working autonomously and making safe, independent clinical decisions.
• Ability to work effectively under pressure and manage a busy workload.
• Good interpersonal and communication skills.

Desirable

• Experience of working as a Practice Nurse, Community Nurse, or Specialist Paramedic.
