Advanced Clinical Practitioner, Frailty/Care of the Elderly Mountain Ash

An excellent opportunity has arisen for an Advanced Clinical Practitioner to join our well established Frailty/Care of Elderly Team. The role will require the successful clinician to work within the four pillars of Advanced Practice which includes:

• Clinical management of patients
• Lead on service developments and change
• Provide education, mentorship and support to a wide multi‑professional audience
• Demonstrate understanding and use of evidence‑based practice and involvement in research (QIP/Audit)

Welsh Skills Desirable: This post is advertised as Welsh Desirable. Whilst the candidate does not need to have skills in Welsh, we will consider it an advantage when short‑listing and selecting candidates. This isn't 'fluency', just speaking & listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.

Main Duties:

• Provide clinical care/management to groups of patients admitted to the Frailty/Care of Elderly service
• Lead on service development
• Provide leadership to MDT members
• Deliver educational packages to a multi‑professional audience
• Maintain own level of education
• Involve in relevant research/audit/SE/QiP's

Job Responsibilities:

• Clinical: Undertake clinical assessment, request relevant investigations, make diagnosis and manage patients according to need, including non‑medical independent prescribing. Undertake regular patient ward rounds as an independent clinician and support patients through their hospital journey from admission to discharge. Support effective communication between ward teams, MDT and patient/relatives. Lead complex discharge processes and ACP‑led discharges. Undertake higher level communication – impart bad news and discuss treatment escalation plans with patients and carers.
• Leadership: Provide clinical leadership of MDT, develop service and projects, attend and be an instrumental part of relevant HB strategy.
• Education: Own education needs, maintain a live portfolio of AP evidence, deliver education within the clinical setting and in formal education settings, provide mentorship and coaching to multi‑professionals undertaking clinical courses such as MSc AP and NMIP.
• Research: Use guidelines to support clinical decisions in practice, involve in audit/research, lead QiP projects.

• Education: Essential – In receipt of relevant clinical Level 7 (Masters) education. Essential – In receipt of non‑medical independent prescribing module. Desirable – Working toward relevant clinical Level 7 (Masters) education. Desirable – Working toward non‑medical independent prescribing module.
• Clinical: Essential – Able to carry out competent physical assessment, make diagnosis and appropriate management plans. Essential – Able to prescribe a wide range of drug treatments safely and independently. Desirable – Working toward being able to carry out competent physical assessment, make diagnosis and appropriate management plans. Desirable – Working toward being able to prescribe a wide range of drug treatments safely and independently.
• Leadership: Essential – Able to demonstrate ability to lead a clinical team. Essential – Able to demonstrate ability to identify and action change in practice. Desirable – Able to understand how to lead a clinical team. Desirable – Able to demonstrate involvement in change processes.
• Research: Essential – Able to demonstrate use or involvement in research in practice. Essential – Able to demonstrate leading or being involved in research‑based projects e.g., QiP. Desirable – Able to demonstrate understanding of research and research‑based projects.
• Skills: Desirable – Welsh Language Skills (Level 3 and above/B1).
