Job summary

Join our Admission Avoidance Team as an Admission Avoidance Clinician and play a vital role in supporting patients to receive the right care, in the right place, at the right time. Working within the Transfer of Care Hub, you will assess patients within urgent and emergency care settings, identify alternatives to hospital admission where appropriate and facilitate safe, timely discharge into community services.

You will work closely with multidisciplinary colleagues across acute, community, primary care and social care services, helping to coordinate care, reduce avoidable admissions and support the delivery of Discharge to Assess and Home First pathways. Main duties of the job

Support the delivery of same-day discharge pathways from Urgent and Emergency Care areas, contributing to improved patient outcomes for individuals with a wide range of health and social care needs, including complex presentations.

Responsible for identification and assessment of patients who may be appropriately supported by community services, contributing to the avoidance of unnecessary admission to acute beds.

Work as part of the Transfer of Care Hub (TOCH), collaborating with multidisciplinary colleagues across acute, community, and primary care settings, as well as Adult Social Care and partner agencies, to support effective partnership working and the coordinated discharge of patients, including those with more complex needs.

Act as a specialist resource within the acute setting, utilising knowledge of community services to support safe, timely, and effective discharge planning, including facilitating access to appropriate services where required.

Ensure patients are appropriately signposted and supported to access follow-on community services, contributing to the reduction of avoidable readmissions and ensuring safe continuity of care.

Ensure accurate, timely, and clinically appropriate communication and handover of care to community services, contributing professional judgement to inform discharge planning and next steps.

Person Specification

Qualifications

Essential

• Current relevant UK Professional Registration
• Relevant health degree or equivalent experience
• Evidence of continuing professional development
• History Taking and Physical Assessment (or equivalent) with demonstrable clinical experience (or willingness to work towards)

Desirable

• Mentorship/Practice Assessor qualification
• Quality improvement or audit training

Experience

Essential

• Post-registration experience within relevant health or social care setting including experience of frailty
• Demonstrable effective communication and interpersonal skills
• Experience of working within a multidisciplinary team
• Experience of managing own workload and contributing to team objectives
• Ability to support delivery of performance targets
• Experience of partnership working across teams/services
• Good written and verbal communication skills
• IT literacy (e.g. Microsoft Office systems)
• Positive, proactive ("can do") approach
• Ability to support and guide junior staff and students
• Willingness to work flexibly to meet service needs

Desirable

• Experience of discharge planning or care coordination
• Involvement in audits or quality improvement initiatives
• Experience supporting service development or change
