This opportunity is currently being offered on a fixed term contract basis of 18 months. If budgets allow, the contract length may be extended and/or become permanent, though this is not guaranteed. We are looking for a candidate with a solution-based approach to housing needs for people who are being discharged from medical or care settings. The role will link housing, social care, and medical services to ensure that discharge plans centre on the transition being made effectively and in a person-focused way. The postholder will establish effective working relations with operational staff and senior stakeholders across housing, NHS and social care. Close work with substance misuse, criminal justice, physical and mental health, primary and secondary care, and voluntary sector agencies will be required to provide a holistic response.

Responsibilities

The postholder will identify barriers preventing discharge from facilities, engage with officers, services and multi-disciplinary teams, and work with partners to coordinate, develop and deliver positive outcomes for service users. By reviewing evidence of best practice for therapeutic interventions, working with service users, and bringing co-production into the process, the postholder will suggest how to improve service pathways for the future. This is an 18-month pilot post that will support making a case for ongoing intervention. The ability to demonstrate how services can be provided to achieve the best outcomes for individuals, and how this can be done to make best use of resources, will be critical. The successful post holder will be subject to an Enhanced DBS check.

These frameworks provide a mechanism for employees to take ownership of their development within their post and be rewarded accordingly. The salary offered will be at level A, with potential progression to levels B and C after in-post development. Person Specification

The person specification of a job profile is split into three levels: Level A details the essential criteria that all applicants must demonstrate through their application and interview. Level B details the developed competencies for a practitioner in this role and represents progression from Level A. Level C requires the highest level of competence, with full demonstration of all aspects of the job profile, including qualifications, knowledge, skills and experience. We are seeking a Complex Care Coordinator to join the team at Level A. This is the entry point of the role, with development supported through induction, probation and a Performance Appraisal and Career Conversation. All Level A elements are essential. Through induction you will be introduced to the CPF to identify strengths and development needs.
