The predicted salary is between 28800 - 43200 £ per year.
We are currently recruiting for a Brokerage Officer to receive and process all requests for placements for home care and day service provision, with a focus on developing a detailed knowledge of provision locally and nationally to ensure that appropriate and high-quality placements are brokered to meet individual need.
Liaise with colleagues from health and IDT to support patients through the hospital discharge process.
Liaise directly with home care and day care providers to discuss potential placements and discuss matching.
Develop a detailed knowledge of specialist provision and their specialisms through liaison with colleagues in the Children and Adults directorate and meeting with providers directly.
Using the detailed knowledge of specialist social care provision to be able to give advice and make recommendations to social workers on appropriate matches.
To work together with colleagues from Brokerage team and the SDS Team to support identification of personalised support to meet the assessed needs.
Update Mosaic case management system with information and details of the placement.
Update integra with information and details of new providers.
To work with social care staff and providers to provide support to placements at risk of breakdown or exclusion and to contribute to analysis of need and cost.
To support negotiation of long-term strategic partnerships with providers to ensure Medway have access to cost-effective high-quality provision for adult Social Care Clients.
