This secondment offers the chance to work autonomously in a specialist nursing role, and make a meaningful impact on patient care. You will provide expert advice and clinical support across community services, supporting staff in the prevention and management of complex wounds and pressure ulcers.

Responsibilities
  • Clinical audits and service development
  • Education and training of community staff
  • Assist and provide advice for patients with complex wounds
  • Implementation of national and local guidelines
  • Provide expert advice and support in the assessment and management of complex wounds, pressure ulcers, leg ulcers, moisture-associated skin damage, and other skin conditions
  • Undertake home visits and joint visits with community nurses and other members of the MDT to assess and review patients with complex tissue viability needs
  • Develop, implement, and evaluate individualised care plans based on current evidence-based guidelines
  • Deliver education and training to staff on wound care, pressure ulcer prevention, skin care, and risk assessment
  • Contribute to induction and ongoing professional development programmes
  • Undertake audits (e.g., pressure ulcer prevalence, documentation, equipment use)
  • Support implementation of national and local guidelines
  • Contribute to policy development and review
Clinical Expertise & Wound Management
  • Assess, diagnose, and manage a wide range of acute and chronic wounds, including pressure ulcers, leg ulcers, diabetic foot ulcers, and surgical wounds
  • Undertake complex wound assessments using evidence-based tools and clinical judgement
  • Recommend appropriate wound care products, dressings, and therapies based on current guidelines
  • Prescribe or advise on compression therapy, negative pressure wound therapy (NPWT), and pressure offloading equipment as appropriate
  • Support end-of-life skin care planning where needed
Pressure Ulcer Prevention & Management
  • Conduct pressure ulcer risk assessments (e.g., using Purpose-T or similar tools)
  • Promote pressure-relieving interventions and liaise with equipment services for appropriate provision
  • Educate staff on repositioning techniques, early signs of skin damage, and skin inspection routines
Education & Training
  • Provide formal and informal teaching to community nurses, care home staff, and allied professionals
  • Support clinical supervision, competency assessments, and mentorship
  • Create or contribute to learning resources, workshops, and in-service training programs
Multidisciplinary Collaboration
  • Act as a point of contact for wound-related advice across the community
  • Work collaboratively with district nursing teams, podiatrists, tissue viability teams, dietitians, GPs, and safeguarding leads
  • Attend and contribute to MDT meetings, case reviews, and safeguarding meetings as needed
Leadership & Service Development
  • Support development and implementation of local wound care guidelines, formularies, and care pathways
  • Contribute to audits, quality improvement projects, and service evaluations
  • Act as a clinical leader in the field of tissue viability, promoting high standards of care and best practice
Patient-Centred Care
  • Empower patients and carers through education and involvement in wound care planning
  • Respect patient dignity, preferences, and cultural considerations in wound care
  • Provide guidance on skin hygiene, nutrition, and lifestyle factors that influence healing
Governance & Record Keeping
  • Maintain accurate and timely documentation in line with NMC standards and local policies
  • Participate in incident reporting, safeguarding alerts, and risk assessments
  • Ensure confidentiality and compliance with data protection regulations
Professional Development
  • Stay updated with national guidelines (e.g., NICE, NWCSP) and emerging wound care evidence
  • Maintain NMC registration and complete required continuing professional development (CPD)
  • Participate in reflective practice, clinical supervision, and annual appraisals
