Overview

We are looking for a registered nurse or paramedic to lead a Neighbourhood Nursing Team in Strood, supporting patients aged 18+ who are housebound in the Medway area. You will assess, manage, plan and deliver care, work within the integrated health team and liaise with GPs, agencies and local authorities.

Responsibilities

  • Lead and manage the Neighbourhood Nursing Team, ensuring a high standard of care for patients and families.
  • Deliver patient care 7am–7pm, 365 days a year; work the following shifts: 7am‑3pm, 8.30am‑4.30pm and 11am‑7pm.
  • Plan, prioritise and organise work to meet demand and maintain caseload balance.
  • Assess, manage and coordinate care for patients allocated to the team, ensuring professional and legal accountability.
  • Co‑operate with GPs, other professionals and statutory and voluntary agencies to provide individualised care.
  • Use technology, research and evidence to inform practice and support patient care.
  • Provide mentoring and support to staff, including students and other healthcare professionals.

Qualifications (Essential)

  • Recognised degree in adult nursing.
  • Registered Nurse on Part 1 of the Nursing Midwifery Council register.
  • Evidence of further study or training qualifications.
  • District Nursing qualification or willingness to commence within 1 year of starting.
  • Full UK driving licence and access to a vehicle.
  • Experience of autonomous practice in community or intermediate care settings.

Qualifications (Desirable)

  • Experience as a nurse prescriber or willingness to undertake training.
  • Post‑graduate qualification in elderly care, chronic disease management or related fields.
  • Knowledge of community nursing, urgent care and admission avoidance.
  • Experience of change management and motivational interviewing.
  • Intermediate IT skills (Windows, Internet, Outlook, Excel, EPEX).
  • Experience of using research evidence to inform practice and audit.
  • Mentoring experience with students or other professionals.
