We are seeking people who are adaptable and collaborative, with the potential to grow, and the ability to plan, prioritise and organise their work, to join our neighbourhood nursing service.
About the Service
Our community nursing service includes district nursing, continence care, neighbourhood nurse practitioners, end of life carers and phlebotomy. It organises, delivers and manages nursing care for patients in their own homes, residential accommodation or clinic settings. We also attend multi-disciplinary team meetings, promote and preserve the health needs of individuals, families and communities, encourage self-care as well as undertake assessment and evaluation of care. The service operates a 7am to 7pm service, 365 days a year.
The community nursing teams provide nursing care for patients aged 18 and over who are housebound within the Medway area, delivering a range of personalised care and treatment to those with complex and long-term conditions, supporting people to stay safe and well at home.
Responsibilities
- Work as a member of a neighbourhood nursing team to deliver person-centred integrated care in the community with the aim of helping people remain independent at home for as long as possible.
- Provide care in line with Buurtzorg principles including \”first coffee then care\”; spend time getting to know patients and their families to enable them to make informed choices.
- Ensure care focuses on improving self-reliance and empowerment, viewing patients holistically and supporting health, social and psychological needs as required.
- Work as an integral part of a team of nursing staff providing consistent care to a defined community caseload 365 days a year, 24/7 as needed, with flexibility and commitment to maintain service provision.
- Collaborate closely with GPs and other health, social care and voluntary organisations including therapists, mental health services, social workers, care navigators and case managers.
- Support the team to achieve core aims: happy patients through direct access, continuity and quality clinical care; happy nurses and caregivers through collaborative and flexible teamwork.
Qualifications and Requirements
- You must have a UK driving licence and access to a vehicle to use for work purposes.
- Adaptable, collaborative, with the potential to grow and the ability to plan, prioritise and organise work.
- Ability to work as part of a neighbourhood nursing team delivering care 365 days a year, 24/7 as required.
