Mediplacements is pleased to be working with a GP practice / Primary Care Network (PCN) in Hertfordshire to recruit an experienced Primary Care Pharmacist for a 6-month fixed term contract.
This role is ideal for a pharmacist with proven experience in Atrial Fibrillation (AF) reviews, QOF, and Enhanced Care Framework (ECF) delivery, looking to make a real impact within primary care.
The Role
- You will support the practice / PCN by delivering high-quality clinical pharmacy services, with a particular focus on AF management and anticoagulation safety.
- Working closely with GPs and the wider multidisciplinary team, you will help improve patient outcomes while supporting the delivery of key contractual targets.
Key Responsibilities:
- Deliver structured Atrial Fibrillation (AF) reviews in line with clinical guidance.
- Review and optimise anticoagulation therapy, including DOACs.
- Support the achievement of QOF and Enhanced Care Framework (ECF) requirements.
- Identify patients requiring intervention to improve safety and outcomes.
- Provide medicines optimisation support and clinical advice to the wider team.
- Maintain accurate clinical records and coding.
- Communicate effectively with patients and healthcare professionals.
Essential Requirements:
- MPharm degree or equivalent.
- Current GPhC registration.
- Demonstrable experience delivering AF reviews in primary care.
- Strong understanding of QOF and ECF frameworks.
- Experience managing anticoagulation, including DOACs.
- Previous experience within a GP practice or PCN.
- Excellent communication and consultation skills.
- Experience using EMIS or SystmOne.
