Mediplacements is pleased to be working with a GP practice / Primary Care Network (PCN) in Hertfordshire to recruit an experienced Primary Care Pharmacist for a 6-month fixed term contract.

This role is ideal for a pharmacist with proven experience in Atrial Fibrillation (AF) reviews, QOF, and Enhanced Care Framework (ECF) delivery, looking to make a real impact within primary care.

The Role

  • You will support the practice / PCN by delivering high-quality clinical pharmacy services, with a particular focus on AF management and anticoagulation safety.
  • Working closely with GPs and the wider multidisciplinary team, you will help improve patient outcomes while supporting the delivery of key contractual targets.

Key Responsibilities:

  • Deliver structured Atrial Fibrillation (AF) reviews in line with clinical guidance.
  • Review and optimise anticoagulation therapy, including DOACs.
  • Support the achievement of QOF and Enhanced Care Framework (ECF) requirements.
  • Identify patients requiring intervention to improve safety and outcomes.
  • Provide medicines optimisation support and clinical advice to the wider team.
  • Maintain accurate clinical records and coding.
  • Communicate effectively with patients and healthcare professionals.

Essential Requirements:

  • MPharm degree or equivalent.
  • Current GPhC registration.
  • Demonstrable experience delivering AF reviews in primary care.
  • Strong understanding of QOF and ECF frameworks.
  • Experience managing anticoagulation, including DOACs.
  • Previous experience within a GP practice or PCN.
  • Excellent communication and consultation skills.
  • Experience using EMIS or SystmOne.
