Are you a great communicator? Do you want a rewarding role helping patients manage their long term health conditions? Enfield Unity PCN is looking for a highly motivated, IT literate, quality aware individual to join our team on a 12 month maternity cover contract. We want someone who can offer support and assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.

The post holder will be confident with EMIS, DOCMAN and Accurx as well as having good Excel skills. They will utilise these skills in contacting groups of patients to book vaccination, immunisation, screening and review appointments across our PCN sites. They will undertake a variety of administrative duties to assist in the smooth running of the Practice including the provision of secretarial and clerical support. They will facilitate effective communication between patients, members of the primary health care team, secondary care and other associated healthcare agencies.

Main duties of the job

We are looking for a Care Co‑ordinator to join our team, working across sites to help improve health outcomes for our patients. You must have excellent communications skills and IT skills as well as patience and understanding, put patient care at the centre of everything you do and support the service as required. Duties may be varied from time to time under the direction of the Lead Clinician/Manager, dependent on current and evolving Practice workload and staffing level. The successful candidate must understand confidentiality as dealing with patients' sensitive information and follow GDPR. Support the equality, diversity and rights of patients, carers and colleagues. The post-holder should recognize the importance of effective communication within the team and strive to maintain quality within the Practice.

IT experience is essential (MS Office & Internet) and Excel. Knowledge of EMIS would be an advantage however full training will be given. Previous experience of working in general practice is preferred and knowledge of the local area is desirable, along with an awareness of the challenges faced in the community. The successful candidate must have or must be willing to complete Safeguarding Certification and training set out by the Personalised Care Institute.

Job responsibilities

The purpose of the role is to:

  • Contact patients to book appointments for reviews of long term health conditions, screening and immunisation or other as required.
  • Maintain records of call/recall activity.
  • Offer general assistance to the Practice team and patients.
  • Project a positive and friendly image to patients and other visitors, either in person or via the telephone.
  • Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
  • Undertake a variety of administrative duties to assist in the smooth running of the Practice including the provision of secretarial and clerical support to clinical staff and other members of the Practice team.
  • Facilitate effective communication between patients, members of the primary health care team, secondary care and other associated healthcare agencies.
Duties and Responsibilities

The duties and responsibilities may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Lead Clinician/Practice Manager, dependent on current and evolving Practice workload and staffing levels.

  • Using the clinical records to identify patients in need of vaccination or immunisation, health advice, screening or review.
  • Contacting by telephone, text or letter.
  • Provide information on the importance of immunisations, screening, review and 'myth-bust' when faced with misinformation.
  • Update patient contact details on the clinical record system.
  • Book appointments.
  • Be aware of the practice philosophy and treat patients/visitors with respect, tolerance and offer constructive support to colleagues and the general public.
  • Maintain and monitor the Practice appointments system.
  • Process appointments; including personal, telephone and online requests for appointments, visits and telephone consultations and ensure patients are directed to the appropriate healthcare professional.
  • Have working knowledge of the telephone systems (during and after hours) and have an appropriate telephone manner.
  • Computer data entry/data allocation and collation; processing and recording information in accordance with Practice procedure.
  • Provide clerical assistance to Practice staff as required from time to time, including word/data processing, filing, photocopying and scanning.
  • Initiate contact with and respond to requests from patients, other team members and associated healthcare agencies and providers.
  • Deal with delegated tasks efficiently, according to guidelines set, and report regularly any problems and targets reached as required.
  • Be aware of all Practice procedures and work in accordance with written protocols.
  • Take messages and pass on information.
  • Work flexibly according to the needs of the Practice and provide cover for absent colleagues.
  • Undertake mandatory and additional training as identified by the practice.
  • Any other duties, appropriate to the grade, as required by the Practice Manager/Lead Clinician.

  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.
  • Actively reporting health and safety hazards and infection hazards immediately when recognised.
  • Keeping own work areas and general/patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holder's role.
  • Undertaking periodic infection control training.

Person Specification

Qualifications:

  • Excellent IT skills including Microsoft Office, Word and Excel.
  • Good general education to GCSE or equivalent, including equivalent grade C or above passes in Maths and English.
  • Experience in a GP Practice environment.
  • Knowledge of EMIS and DOCMAN.
