Role: Band 6 Community Occupational Therapist (Adult Rehab)
Location: Wales
Rate: Up to 40 per hour (Umbrella), DOE
Contract: Part-time | 8 weeks initially

Responsibilities:
- Community adult rehab caseload
- Functional assessments, goal-focused treatment plans
- Reablement, equipment/adaptations, safe discharge planning
- MDT working across community settings
Requirements:
- HCPC registered OT
- Recent Band 6 community rehab experience (NHS preferred)
- Confident with home visits and risk assessments
- UK RTW + enhanced DBS
- Car driver
