Job Title: Home First Team, Social Worker (Adults)

Salary: £35.00 per hour

About The Role
The primary focus of these posts is to adopt a home first or discharge to assess way of working to support people in returning home from hospital safely and promptly. You will be an integral part of a successful partnership between health, social care, and our home, independence, and crisis support (HICS) team.

What will you be doing?

  • Attend acute and community hospitals across Dorset to support and facilitate discharge.
  • Come into the office to support duty once a week.
  • Lead social work assessments for complex hospital discharges, ensuring timely planning in line with national and local guidance.
  • Undertake comprehensive social care assessments under the Care Act 2014, focusing on individuals\’ strengths and desired outcomes.
  • Conduct risk assessments to ensure safe discharge and community safety, escalating concerns as needed.
  • Develop and review care plans, coordinating services like reablement, equipment, and long-term care.
  • Collaborate with NHS staff, GPs, housing providers, voluntary organizations, and others for seamless care transitions.
  • Identify and respond to safeguarding concerns per Dorset Council policies.
  • Apply the Mental Capacity Act 2005 principles, conducting assessments and making decisions in partnership with individuals.
  • Manage caseloads effectively, prioritizing urgent discharge work.

About You
You must hold a recognised Social Work qualification and be registered with Social Work England.
Experience in hospital discharge settings and handling complex cases is preferred.
Enhanced DBS clearance (Children’s & Adults) will be obtained.
Right to work in the UK required.
